Objective:To study on the clinical effect of mifepristone combined with modified B-lynch suture in the treatment of central placenta previa.Method:Clinical data of 52 women suffering from central placenta previa between August 2009 and December 2011 in a hospital of Africa were retrospectively analyzed.The patients were divided into the study group (n =29)and the control group (n =23).The study group was treated with mifepristone combined with modified B-lynch suture,and the control group used the conventional therapy.The operation time,the amount of bleeding during the operation,the number of uterine resection,the amount of vaginal bleeding two hours after the operation and the incidence of complications were evaluated.Results:The operation time were (52.7 ± 15.8) mins for the study group and (68.7 ± 17.1) mins for the control group (P ＜0.05).The amount of bleeding during the operation were (495.2 ± 164.6) ml for the study group and (742.7 ±215.8) ml for the control group (P ＜ 0.05).Seven patients experienced uterine resection in the control group and none in the study group (P ＜ 0.05).The amount of vaginal bleeding two hours after the operation were (52.4 ± 6.3) ml in the study group and (63.5 ± 8.9) ml in the control group (P ＜ 0.05).Complications were found in one patient in the study group and 7 ones in the control group (P ＜ 0.05).Conclusion:Mifepristone combined with modified B-lynch suture in the treatment of central placenta previa could reduce the amount of postoperative bleeding and the incidence of uterine resection.It should be spread in clinical use..%目的:研究米非司酮联合改良式B-lynch缝合术治疗中央性前置胎盘的临床疗效.方法:对非洲马里马尔格拉医院2009年8月～2011年7月收治的中央性前置胎盘52例病历资料进行回顾性分析,根据治疗方法不同分为观察组29例和对照组23例.观察组采用米非司酮联合改良式B-lynch缝合术治疗,对照组采用常规方法治疗,治疗结束后观察两组手术时间、术中出血量、子宫切除、术后2h阴道流血量和并发症等指标.结果:手术情况观察组和对照组分别为手术时间52.7±15.8min、68.7±17.1min,术中出血量495.2±164.6ml、742.7±215.8ml,子宫切除病例0、7例(7/23),术后2h出血量52.4±6.3ml、63.5±8.9ml,发生并发症1例(1/29)、6例(6/23),两组比较具有统计学差异(P＜0.05).结论:米非司酮联合改良式B-lynch缝合术治疗中央性前置胎盘剖宫产术中术后出血量低,并发症少,减少子宫切除病例发生,值得在临床推广应用.
