体外反搏治疗冠状动脉粥样硬化性心脏病的疗效观察

目的：观察体外反搏治疗对冠状动脉粥样硬化性心脏病（冠心病）患者的疗效。方法选择2014年3月~2015年6月在德阳市第六人民医院住院的冠心病患者60例，其中男性33例，女性27例，年龄46~78岁，平均（67.4±8.4）岁。将患者按照随机平等原则分为体外反搏组（30例，采用体外反搏+冠心病常规药物治疗）和对照组（30例，只采用冠心病常规药物治疗）。治疗3个疗程后评定临床疗效和心电图疗效，治疗前后检测心电图和测定左室射血分数，记录胸痛发作次数。结果体外反搏组临床疗效显效17例（56.7%），有效12例（40.0%）；对照组显效12例（40%），有效10例（33.3%）。体外反搏组临床疗效总有效率高于对照组（96.7%vs.73.3%），差异有统计学意义（P＜0.01）。体外反搏组心电图疗效总有效率高于对照组（93.3%vs.70.0%），差异有统计学意义（P＜0.01）。两组治疗后胸痛发作次数、ST段下降幅度、左室射血分数较治疗前均改善，差异有统计学意义（P均＜0.05）。与对照组治疗后比较，体外反搏组胸痛发作次数、ST段下降幅度、左室射血分数均改善，差异有统计学意义（P均＜0.05）。结论冠心病患者采用体外反搏治疗较常规治疗疗效更显著。%Objective To observe the curative effect of external counterpulsation in treatment of coronary heart disease (CHD).Methods CHD patients (n=60, male 33, female 27, aged from 46 to 78 and average age=67.4 ±8.4) were chosen from Mar. 2014 to Jun. 2015. All patients were randomly divided into external counterpulsation group treated with external counterpulsation and CHD routine drugs and control group treated with only CHD routine drugs (eachn=30). After 3 therapeutic courses, the curative effect and electrocardiogram (ECG) outcomes were reviewed. ECG and left ventricular ejection fraction (LVEF) were detected and attack times of chest pain were recorded before and after treatment.Results There were 17 cases (56.7%) with significant efficacy and 12 (40.0%) with curative effect in external counterpulsation group, and 12 (40%) with significant efficacy and 10 (33.3%) with curative effect in control group. The total effective rate was higher in external counterpulsation group than that in control group (96.7%vs. 73.3%,P<0.01). The total effective rate of ECG outcomes was higher in external counterpulsation group than that in control group (93.3%vs. 70.0%,P<0.01). After treatment, the attack times of chest pain, ST-segment descending latitude and LVEF were improved in 2 groups after treatment (allP<0.05). Compared with control group after treatment, the attack times of chest pain, ST-segment descending latitude and LVEF were improved in external counterpulsation group (allP<0.05).Conclusion The curative effect of external counterpulsation is more significant than that of routine therapy in CHD patients.
