中西药结合治疗肺癌放疗致放射性肺炎32例疗效观察

摘要

Objective To study the therapeutic effect of combination of the traditional Chinese and western medicine in treatment of lung cancer with radiotherapy radiation pneumonia and effects on pulmonary function indexes. Methods Sixty two patients who were diagnosed with radiotherapy radiation pneumonia, were randomly divided into two groups, thirty two cases in treatment group, who were administered hormone and antibiotics,combined with oxygen,cough relieving,antispasmodic, symptomatic treatment, while were treated with Qingfei lily decoction. Thirty cases in control group, in addition to accept Qingfei lily Decoction, other therapy were same as treatment group. To observe the clinical efficacy and changes of clinical symptom score, improvement of quality of life, pulmonary function, blood gas analysis, peripheral hemogram in 2 groups before and after treatment. Results After the treatment, the total therapeutic effects and the effective rate of KPS in the treatment group was better than the control group( P <0. 05 ); symptoms score in both group decreased after the treatment, comparing with the same group before the treatment, with the difference was statistical significance ( P <0. 01 ); the treatment group was superior to those of the control group( P <0. 05 ); After treatment, pulmonary function, blood gas analysis, blood indexes were improved, but the treatment group improved obviously( P < 0. 05 ). Conclusion Compared with the single western medicine treatment, Qingfei lily decoction combined with western medicine in treatment of radiation pneumonia can strengthen the function of controlling infection and inflammatory reaction, improve the pulmonary function and blood gas status, the symptoms, and quality of life of patients with radiation pneumonia, control of the disease.%目的 观察中西药结合治疗肺癌放疗致放射性肺炎的疗效及对肺功能等指标的影响.方法 将62例放射性肺炎患者随机分为2组,对照组30例给予口服联合激素与抗感染治疗,辅以吸氧、止咳、解痉平喘等对症处理.治疗组32例在对照组基础上加用清肺百合汤.2组均4周为1个疗程观察临床疗效和治疗前后2组生活质量、临床症状积分改善情况,以及治疗前后肺功能、血气分析、外周血象等指标的变化.结果 治疗组放射性肺炎总有效率、生存质量KPS评分改善总有效率显著优于对照组(P﹤0.05);与治疗前比较,治疗后2组症状积分均明显下降(P﹤0.01);治疗组优于对照组(P﹤0.05);治疗后2组肺功能指标(VC、FEV1)、血气分析指标(PaO2、PaCO2)、血象(WBC、N)均有好转(P<0.05,P<0.01),但治疗组较对照组改善更明显(P﹤0.05).结论 清肺百合汤联合西药治疗可增强控制感染及机体炎性反应、改善肺功能和血气状况的作用,较单纯西药治疗能更好地改善放射性肺炎患者的症状,提高患者生存质量,控制病情.
