辅酶Q10联合瑞舒伐他汀对慢性心力衰竭患者心功能及血液TNF-α、IL-6、BNP水平的影响

摘要

目的 观察辅酶Q10联合瑞舒伐他汀对慢性心力衰竭患者心功能及肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、脑钠肽(BNP)等的影响.方法 选择2015年6月-2016年10月湖北省中医院心血管内科接受治疗的慢性心力衰竭患者92例作为研究对象,将其采用随机数字表法分为观察组和对照组,各46例.2组患者均接受常规治疗,包括β受体阻滞剂、血管紧张素转化酶抑制剂、利尿剂以及强心类药物等.在常规治疗基础上,对照组患者加用瑞舒伐他汀治疗,观察组患者在对照组基础上加用辅酶Q10治疗.2组均连续治疗8周,比较2组患者的临床治疗效果及治疗前后的心功能指标、TNF-α、IL-6、BNP等水平的变化情况.结果 治疗后,观察组总有效率为93.5％,高于对照组的78.2％(χ2=4.390,P=0.036);2组心功能指标LVEF、LVEDD、LVESD均较治疗前显著改善(P<0.01),且观察组患者改善优于对照组(t=7.230、3.875、4.620,P均=0.000);2组TNF-α、IL-6、BNP水平均较治疗前显著下降,并且观察组患者下降程度大于对照组(t=4.792、7.542、46.008,P均=0.000).结论 辅酶Q10联合瑞舒伐他汀治疗慢性心力衰竭能够获得良好的临床效果,显著改善患者的心功能指标及炎性介质水平,具有重要的临床意义,值得加以推广.%Objective To observe the effects of coenzyme Q10 combined with statins on cardiac function and TNF-α,IL-6,BNP in chronic heart failure.Methods 92 cases of patients with chronic heart failure were selected,who were treated in department of cardiology of Hubei Provincial Hospital of Traditional Chinese Medicine from June 2015 to October 2016,and they were divided into observation group (46 cases) and control group (46 cases).The patients of two groups were all treated with routine treatment,including beta receptor inhibitors,angiotensin converting enzyme inhibitors,diuretics,and cardiac drugs.The patients of control group were treated with rosuvastatin.Based on the treatment of the control group,the patients of observation group were treated with coenzyme Q10.Observe and compare the clinic effect and changes of heart function index,TNF-α,IL-6,BNP levels of the two groups.Results After the appropriate treatment,the patients of the observation group got a total effective rate of 93.5％,which was significantly better than the 78.3％ of the control group (χ2=4.390,P=0.036);The LVEF,LVEDD and LVESD levels of two groups were all significantly improved,and the improvement in the observation group was more significant (t=7.230,t=3.875,t=4.620,P=0.000,P=0.000,P=0.000);The TNF-α,IL-6,BNP levels of two groups were all significantly decreased,and the decline in the observation group was more significant (t=4.792,t=7.542,t=46.008,P=0.000,P=0.000,P=0.000).Conclusion Coenzyme Q10 combined with rosuvastatin in the treatment of chronic heart failure can achieve good clinical results,significantly improve the patient's heart function and inflammatory mediators,which has important clinical significance and deserves promotion.
