增加运动量对新诊断肥胖2型糖尿病患者影响的观察

         

摘要

Objective To investigate the effects of different exercise intensities on metabolism, β-cell function, and insulin sensitivity (IS) in newly diagnosed obese T2DM patients. Methods Seventy-four newly diagnosed obese T2DM patients were randomized into two groups: treated with exercise for 200—250 minutes per week (intensive group) and for 100~150 minutes per week (control group) respectively. Both groups were given diabetes propaganda, diabetic diet management, and small dose of metform-in (0. 5 g/d). The values of blood lipid, blood pressure, BMI, FBG, Fins, FC-P, HbA1c HOMA-IR, HOMA-β, and AI30/AG30 were determined and compared after 8 and 16 weeks' treatment Results Compared with the control group, the level of TG, TC, HbA1c and HOMA-IR were decreased significantly in the intensive physical activity group (P<0. 05 or P<0. 01) after 8 weeks. After 16 weeks, the OGTT test was performed and the glucose level at 0 min and 60 min in the intensive physical activity group were significantly lower than in the control group (P<0. 05 or P<0. 01); the level of TG, DBP, BMI, HbA1c, and HOMA-IR were significantly lower (P<0. 05 or P<0. 01), however, the level of HDL-C, FC-P, and HOMA-β were increased significantly (P<0. 05 or P<0. 01) compared with the control group (P<0. 05 or P<0. 01). Conclusions Increasing the intensity of physical activity will be beneficial to the improvement of metabolism indices and (3-cell function in newly diagnosed T2DM patients.%目的 观察不同运动水平对新诊断肥胖T2DM患者代谢指标、胰岛功能及IS的影响.方法 将74例新诊断肥胖T2DM患者随机分为强化运动组(强化组)及一般运动组(对照组),两组均予糖尿病知识健康教育、饮食管理、小剂量二甲双胍(0.5 g/d);强化组运动200~250min/周,对照组运动100~150min/周.干预8周及16周后对两组血脂、血压、BMI、FBG、FIns、FC-P、HbA1c、胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMA-β)及胰岛素初期相分泌指数(△I30/△G30)进行比较. 结果干预8周后强化组TG、TC、HbA1c、HOMA-IR明显低于对照组(P<0.05或P<0.01);16周后强化组0 min及60 min血糖、TG、DBP、BMI、HbA1c,以及HOMA-IR明显低于对照组(P<0.05或P<0.01);HDL-C、FC-P及HOMA-β明显高于对照组(P<0.05或P<0.01). 结论 新诊断肥胖T2DM患者通过增加运动量有益于代谢指标及胰岛功能的进一步改善.
