目的:观察肿瘤坏死因子-α(TNF-α)在牙周组织及肺组织中的表达及变化,从细胞因子的角度探讨牙周炎和慢性肺感染的相互关系.方法:将30只昆明小鼠随机平均分为A组和B组,随机选取其中一组(A组)建立小鼠慢性肺感染动物模型后,将A组小鼠随机分为3个亚组,在原有肺感染的基础上叠加建立牙周炎动物模型,即A1组(慢性肺感染+牙周正常组)、A2组(慢性肺感染+轻度牙周炎组)、A3组(慢性肺感染+重度牙周炎组).B组小鼠亦分为3个亚组,建立牙周炎动物模型,即B1组(正常对照组)、B2组(轻度牙周炎组)、B3组(重度牙周炎组),采用免疫组化染色观察TNF-α在各组牙周组织及肺组织中的表达,应用计算机图像分析系统半定量分析TNF-α的阳性表达水平.结果:牙周袋深度、牙槽骨吸收总量、肺灌洗液淋巴细胞百分数及牙周组织和肺组织中TNF-α的平均光密度值(OD)在A组各亚组间及B组各亚组间的差异均有统计学意义(P<0.05),且A3>A2>A1、B3>B2>B1.而在A1与B1、A2与B2、A3与B3的比较中牙周袋深度、牙槽骨吸收总量及牙周组织中TNF-α平均光密度值差异无统计学意义(P>0.05).结论:TNF-α可能参与了牙周炎和慢性肺感染的发病过程,且在牙周炎与慢性肺感染的关系中发挥作用.%AIM: To evaluat the expression of TNF-a in the periodontal tissues and the lung tissues, in or-der to observe the relationship of TNF-a expression with periodontitis and chronic lung infection. METHODS: Thirty Kunming mice were randomly divided into 2 groups. Group A was used to establish chronic lung disease model. Group B was used to establish chronic periodontitis model. Group A was further divided into group Al (chronic lung disease with no periotontitis) , group A2 (chronic lung disease with mild periotontitis) and group A3 (chronic lung disease with severe periotontitis). Group B was further divided into group Bl (control group) , group B2 ( mild periotontitis) and group B3 (severe periotontitis). Immunohistochemistry were used to evaluate the expression of TNF-a in the periodontal tissues and the lung tissues. Computer image analysis system was used for semi-quantitative analysis of the image. RESULTS: The intensity of TNF-a expression was in the order of A3>A2>Al and B3>B2>Bl. Significant difference of TNF-a expression was found among different subgroups within the same group (P <0.05). However, no significant difference of TNF-a expression was found between groups A1 and B1, A2 and B2, A3 and B3 (P > 0.05). CONCLUTION: TNF-a may play a role in the development of periodontitis and chronic lung infection.
