空气喷磨结合酸蚀对预防性树脂充填后微渗漏的影响

         

摘要

AIM:To compare the effects of cavity preparation between air-abrasion and traditional hand-piece method. METHODS:100 extracted premolars and molars were divided into Group A,B,C,D and E(n=20). The cavity preparation in group A,B and C was conduccted by trraditional handpiece,in group D and E by air-abra-sion. The teeth of group A were managed by acid etching and resin adhesive before composite resin filling,then smear fissure sealant was applied on the surface;the teeth of group B and D were managed by acid etching and resin adhesive before 3M Z350 filling;the teeth of group C and E were filled with Constic. The microlea-kage and the gap between tooth tissue and the filling material were measured by methylene blue staining and steoriomicroscope respectively. RE-SULTS:Microleakage level and gaps were A>C>E>B>D. Group D vs other groups,P<0.05;group B and E vs group C and A(P<0.05);group B vs E,P>0.05;group C vs A,P>0.05.The gap of group A was larger than that of group C(P<0.05). CONCLUSION:Air-abrasion combined with acid etching,resin adhesive and 3M Z350 are more effective than traditional handpiece method in the preventive resin restoration.%目的:观察用传统涡轮机和空气喷磨术处理窝沟龋及流体树脂充填后,充填体边缘微渗漏情况.方法:选择患有窝沟龋的前磨牙或磨牙100个,随机分为A、B、C、D、E 5组(n=20).A、B、C组用传统涡轮机备洞,D、E组用空气喷磨备洞;A组酸蚀并涂布粘结剂后,用纳米复合树脂充填,最后涂布一层窝沟封闭剂;B、D组酸蚀并涂布粘结剂后,用Z350流体树脂充填;C、E组直接用Constic 树脂充填.然后将各组样本进行冷热循环后,用亚甲蓝染色,体视显微镜观察各组微渗漏情况,并用扫描电镜观察充填体与牙体组织的间隙.结果:5组微渗漏程度为A组>C组>E组>B组>D组,D组与其他4组相比均有统计学差异(P<0.05);B、E组的微渗漏均低于A、C组(P<0.05);B组和E组、C组和A组的微渗漏则无统计学差异(P>0.05).5组充填体与牙体组织间隙宽度为A组>C组>E组>B组>D组,除B、E组相比无统计学差异(P>0.05)外,其余各组两两相比均有统计学差异(P>0.05).结论:空气喷磨技术结合全酸蚀粘结系统,并用Z350流体树脂进行预防性树脂充填能够获得更好的防止微渗漏的修复效果.
