一种用于大鼠肾移植模型的输尿管体外结扎技术

         

摘要

目的 探讨以输尿管体外结扎(Extracorporeal ligation ofureter,ELU)取代肾移植受体大鼠原肾切除的可行性。方法 观察ELU对非移植组(组1)大鼠的影响;比较ELU(组2)与原肾术中切除(组3)及原肾术后切除(组4)对肾移植受体大鼠存活及术后早期肾功能的影响。结果 组1大鼠左侧ELU后15d内均存活良好,右侧ELU后均在3~4d内死亡;组2ELU后第3dSCr明显低于术后第3d;组3的SCr及组4再次手术切除原肾后第3d的SCr〔(46.6±4.4)μmol/Lvs(68.1±7.3)μmol/L,P<0.001及vs(54.3±4.6)μmol/L,P<0.005〕;术后15d内组2大鼠存活率明显高于组3及组4(100%vs60.0%及vs57.2%,P<0.05);病理检查显示ELU后无漏尿、感染并发症。结论 输尿管体外结扎技术方法简便、效果可靠、创伤小,可取代肾移植受体大鼠原肾切除。%Objective To explore the feasibility of replacin g nep hrectomyof the recipient native kidney with extracorporeal ligation of ureter(E LU) for kidney transplantation in rats.Methods The effects of ELU on rats were observed in 16 rats of nontransplant group(g roup 1).The influence of ELU(group 2) and nephrectomy of the native kidney duri ng transplantation(group 3) or after transplantation(group 4) on survival and ea rly renal function of the recipients was compared.Results The rats in group 1 survived well till 15days after ELU of the le ft ureters and died within 3 to 4 days after ELU of the right ureters.Compared with group 3 and group 4 on the 3rd day of nephrectomy of the native kidney,the serum creatinine (SCr) of group 2 on the 3rd day after ELU was lower significantly 〔(46.6±4.4) μmol/L vs (68.1±7. 3)μmol/L and vs (54.3±4.6)μmol/L,p<0.001 and p<0.005 respectively〕.The surv ival rate of group 2 was higher than that of group 3 and group 4 significantly ( 100% vs 60.0% and vs 57.2%,p<0.05).There were no leakage of urine and infection at the site of ligation in all ELU rats. Conclusion The EL U technique is simple,reliable and causes little trauma to rats,thus it can be used to replace the nephrectomy of the recipient native kideny for kidney trans plantation in rats.
