老年人大疱性类天疱疮与内科疾病关系的研究

摘要

Objective To investigate the relationship between bullous pemphigoid (BP)and internal medicine diseases in the elder patients.Methods The clinical data of 23 elder BP,11 adult BP and 23 elder patients with other skin diseases in General Hospital of Air Force PLA during 2008.O1-2010.10 were collected and analyzed by retrospective case-control method.Results The frequency of neurological diseases in elder BP( 43.5％ ) was higher than that in adult BP(9.1％ ,P =0.06)and in elder non-BP(8.7％ ,P ＜0.01 ).Logistic regression analysis showed that the elder patients with neurologic diseases were at the risk of BP occurrence than patients without neurologic diseases( OR = 6.41 ,P = 0.03 ).The bed confinement happened only in the BP patients with nervous system disease.The nervous system diseases are mainly dementia, cerebral vascular disease and Parkinson' s disease.Conclusion Our results indicate that neurodegenerative disease and stroke are major risk factors for elder BP patients, and BP is associated with the severity of neurologic diseases.%目的 探讨老年大疱性类天疱疮(BP)与内科疾病的关系.方法 收集住院的23名老年和11名成年BP及23名老年其他皮肤病患者的临床数据进行回顾性病例对照分析.结果 神经系统疾病在老年BP组出现的频率(43.5%)数值明显高于成年BP组(9.1%),但差异无统计学意义(P=0.06),也高于老年非BP组(8.7%,P＜0.01),Logistic回归显示具有神经系统疾病的老年人患BP的风险高于无神经系统疾病的老年人(OR＝6.41,P=0.03).卧床仅发生于合并神经系统疾病的BP患者.BP患者所合并的神经系统疾病主要为痴呆、脑血管病和帕金森病.结论 神经系统病变和脑血管病是老年BP的独立危险因素,且BP与神经系统疾病的严重程度有关.
