目的 研究初诊2型糖尿病(T2DM)患者胰腺脂肪沉积程度以及与胰岛素抵抗(IR)的关系.方法 收集对照组(30例),T2DM非肥胖患者(20例),T2DM肥胖患者(30例),均接受上腹部CT检查,计算脾胰CT差值,评估胰腺脂肪沉积的程度.结果 胰腺脂肪沉积程度在对照组[(4.29±3.39) Hu],T2DM患者[(8.56±6.15) Hu],T2DM肥胖患者[(8.10±5.82)Hu]内呈递增趋势(P＜0.01),胰腺脂肪沉积是胰岛素抵抗指数(HOMA-IR)的独立相关因素(P＜0.01).结论 胰腺脂肪沉积与IR相关,可能参与T2DM的发展.%Objective To investigate the relationship between the fat deposition in the pancreas in newly diagnosed type 2 diabetes mellitus (T2DM) patients. Methods Fiftity newly diagnosed T2DM patients (including twenty fat patients), thirty healthy cases served as control were selected. All subjects had received upper abdominal CT scan and Hounsfield units (Hu) were measured. The differences between the mean values of spleen and pancreas were calculated and the extent of fat deposition in the pancreas were assessed. Results Compared to the control group(4.29 ±3. 39 Hu) ,the degree of fat deposition in the pancreas in T2DM group (g. 36 ±6. IS Hu) and fat T2DM (8.10 ±5.82 Hu) increased significantly (P <0.01). The degree of fat deposition in the pancreas was the independent relevant factors of insulin resistance index (HOMA - IR) (P <0.01). Conclusion The fat deposition in the pancreas may be involved in insulin resistance,and is closely related to T2DM.
