维吾尔族、汉族小儿先天性心脏病患者围术期凝血功能的比较

摘要

目的 观察新疆维吾尔族和汉族先天性心脏病小儿体外循环下凝血指标的变化差异.方法 随机选取我院确诊为先心病的维吾尔族、汉族患儿各75例.于术前、出室、术后24 h3个时间点分别采血,观察两组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、凝血酶时间(TT)、血小板(PLT)的变化,以及手术时间、体外循环时间、24 h胸腔引流量、浓缩红细胞用量、血浆用量等一般资料.结果 维吾尔族、汉族小儿不同时间点PT、APTT、FIB、TT、PLT差异均无统计学意义(均为P>0.05).维吾尔族组术后24h胸腔引流量[(132.12±12.23) ml比(89.25±12.15)ml,t=2.521,P=0.015],浓缩红细胞用量[(156.08±17.25) ml比(102.12±12.21) ml,t=1.285,P=0.020]、血浆用量[(180.15 ±15.13)ml比(110.28±20.25)ml,t=2.214,P=0.038]均大于汉族组小儿.结论 新疆维吾尔族和汉族先天性心脏病小儿体外循环下凝血指标差异无统计学意义.但术后24h胸腔引流量,浓缩红细胞用量以及血浆用量维吾尔族小儿多于汉族小儿.%Objective To observe the changes of blood coagulation indexes between Uygur and Han children with congenital heart disease in Xinjiang undergoing cardiopulmonary bypass.Methods The 75 Uygur children and 75 Han children with congenital heart disease were randomly selected from our hospital.Blood samples were collected at preoperative,immediately and 24 h postoperation.Prothrombin time (PT),activated partial thromboplastin time (APTT),fibrinogen (FIB),thrombin time (TT) were measured before and 3 hours after operation.Platelet(PLT) changes,as well as operation time,cardiopulmonary bypass time,24-hour chest drainage,concentrated red blood cells,plasma dosage and other general information were compared between the two groups.Results There were no significant differences in PT,APTT,FIB,TT,PLT between Uygur and Han children at different time points (all P > 0.05).Postoperative 24 h chest drainage [(132.12 ± 12.23) ml vs.(89.25 ± 12.15) ml,t =2.521,P =0.015],concentration of red blood cells [(156.08 ± 17.25) ml vs.(102.12 ± 12.21) ml,t =1.285,P =0.02],plasma dosage [(180.15 ± 15.13) ml vs.(110.28 ± 20.25) ml,t =2.214,P =0.038] in Uygur children were significantly higher than those of Han children (all P < 0.05).Conclusions There were no significant differences in blood coagulation indexes between Uygur and Han nationality children with congenital heart disease undergoing cardiopulmonary bypass.However,24 h postoperative,chest drainage,the amount of red blood cells and plasma infusions were higher in Uygur children than those in Han children.
