门诊患者食物诱发严重过敏反应的病因及临床特征

         

摘要

Objective To investigate the clinical characteristics and aetiologyic factors of food-induced anaphylaxis among Chinese outpatients.Methods Medical records of outpatients who were diagnosed as anaphylaxis were collected from Department of Allergy, Peking Union Medical College Hospital since June 18th, 2008 to July 1st, 2012. The allergenic foods and other clinical characters were analyzed retrospectively.Results 170 patients were diagnosed as anaphylaxis during the period mentioned above, 97 of them were involved in this study whose medical records were stored in PUMC hospital.77 patients were diagnosed as food-induced anaphylaxis, whose average age was 28 ( 13 to 61 ) years-old.Patients were divided into adolescent (13-17 years-old) and adult ( above 18 years-old) group.Allergenic foods could be identified among 89.6% of patients.Wheat ( 53.2%) was the most common allergenic food among all patients, whereas vegetables/fruits ( 57.1% vs.26.8%) and legume/peanut ( 52.4% vs. 21.4%) were more common among adolescents compare to adults group ( P<0.05) .Percentage of pollen sensitization was significantly higher in the adolescents group ( 71.4% vs.32.1%, P <0.05 ) compare to adults group.Cutaneous symptoms were the most common (80.5%) clinical manifestation, followed by respiratory symptoms ( 57.1%) .Among the patients with wheat-induced anaphylaxis, the percentage of exercise-induced was significantly higher ( 31.7%, P<0.05 ) compare to patients whose anaphylaxis were induced by the other kinds of foods.Chronic urticaria/dermatitis seemed to be the risk factors for developing life-threatening anaphylaxis (29.5%vs.9.1%, P<0.05).Conclusion In our study, wheat was the most common allergenic food among all outpatients, and often induced by exercise. Cutaneous symptoms were the most common symptoms.Vegetables, fruits, legume and peanut should be aware of potential allergenic foods for food-induced anaphylaxis among hay fever adolescent patients.High risk for life-threatening anaphylaxis should be conscious of anaphylaxis patients with chronic urticaria/dermatitis.%目的:分析食物严重过敏反应患者的变应原及临床特征,以提高诊断此类疾病的水平。方法回顾性检索北京协和医院2008年6月18日至2012年7月1日门诊就诊的严重过敏反应者,查留档病案,分析致敏变应原,并总结其临床发病特征。结果电子门诊系统回顾性检索严重过敏反应者,共检出170例,留有大病历者97例,进行病因分析,其中食物诱发为77例,中位年龄28岁,青少年组(13~17岁)共21例(27.3%),成人组(≥18岁)共56例(72.7%)。能明确具体食物诱因者69例,占89.6%。最常见的食物诱因为小麦,占53.2%;与成人组比较蔬菜/水果(57.1% vs.26.8%)及豆类/花生(52.4% vs.21.4%)作为食物诱因的比例在青少年组显著升高(P<0.05)。同时,青少年患者并发吸入物致敏显著高于成人(95.2% vs.51.8%, P<0.05),主要为花粉致敏比例明显升高(71.4% vs.32.1%, P<0.05)。临床表现以皮肤症状最常见,占80.5%;其次为呼吸道症状,占57.1%。小麦过敏引起的严重过敏反应中,运动诱发比例明显升高(31.7%, P<0.05)。存在慢性荨麻疹和(或)皮炎基础病史的患者出现重度反应的比例更高(29.5% vs.9.1%, P<0.05)。结论食物严重过敏反应的门诊患者中,最常见的诱因为小麦,常由运动诱发。皮肤症状最常见,其次为呼吸道症状。对于青少年花粉症患者,应注意提醒观察有无明确的植物类食物过敏症状。既往存在慢性荨麻疹/皮炎的患者应嘱其警惕严重过敏反应发作症状。
