象皮生肌膏联合VSD对下肢慢性皮肤溃疡术前伤口床准备的临床价值

摘要

Objective To explore the clinical value of Xiangpi Shengji Paste combined with vacuum sealing drainage(VSD) in preoperative wound bed preparation of skin graft/flap of patients with chronic skin ulcer in lower extremity. Methods A total of 113 cases of patients with chronic refractory skin ulcer in lower limb were enrolled in this study, were randomly divided into observation group, Xiangpi Shengji Paste treatment group and VSD treatment group. The observation group used Xiangpi Shengji Paste+VSD treatment, Xiangpi Shengji Paste treatment group with Xiangpi Shengji Paste treatment, VSD treatment group were treated with VSD, compared three groups of patients with debridement and wound bed preparation times required time. Results After the first debridement, the pain, wound odor and wound redness disappeared gradually, and the wound pain and the redness and swelling disappeared fastest in the observation group, and the effect was most obvious. The number of debridement and wound bed preparation in observation group was less than that in other groups, and the difference was statistically significant(P<0.01). Conclusion Xiangpi Shengji Paste combined with vacuum sealing drainage has several advantages, including less times and duration of debridement in preoperative wound bed preparation of skin graft/flap of patients with chronic skin ulcer in lower extremity and better base. It has higher application value, which is worthy of clinical promotion.%目的:探讨象皮生肌膏联合负压封闭引流技术(vacuum sealing drainage,VSD)在下肢慢性皮肤溃疡皮片/皮瓣术前伤口床准备的临床价值.方法:将符合纳入标准的113例下肢慢性难愈性皮肤溃疡患者作为研究对象,随机分为观察组、象皮生肌膏治疗组和VSD治疗组.观察组用象皮生肌膏+VSD治疗,象皮生肌膏治疗组采用象皮生肌膏外敷治疗,VSD治疗组单纯应用VSD治疗,比较三组患者清创次数及皮片/皮瓣术前伤口床准备所需时间.结果:所有患者创面第一次清创后痛觉、创面异味、创周红肿逐渐消失,观察组创面疼痛感和创周红肿消失最快,效果最明显;观察组清创次数及伤口床准备所需时间相对较少,差异有统计学意义(P<0.01).结论:象皮生肌膏联合VSD对慢性难愈性皮肤溃疡创面术前伤口床准备具有其独特优势,能快速清除创面坏死组织,促进创面肉芽生长,提供良好创基,临床应用价值较高,值得推广应用.
