皮肤缺损皮瓣移植术后高压氧早期干预治疗的临床观察

     

摘要

Objective To evaluate the clinical effect of hyperbaric oxygen (HBO) in early stage of skin flap transplantation.Methods In our hospital from January 2011 to August 2013, the skin defect of skin flap transplantation in 50 cases, early application of hyperbaric oxygen therapy on the basis of hyperbaric oxygen therapy nursing intervention were divided into control group and observation group of 25 patients in control group received routine care of patients, the observation group in the control group on the basis of intensive nursing intervention, following 10d observation and assessment, clinical effect of the treatment of skin symptoms and cosmetic satisfaction.Results The observation group and the control group clinical curative effect and skin flap necrosis area of comparison, the difference was statistically significant (P<0.05).The growth status of the 10d flap postoperative score and cosmetic satisfaction were higher than the control group, the difference was statistically significant (P<0.05).In the observation group, the change of skin ruddy time and the time of swelling and exudation were lower than those of the control group, the difference was statistically significant (P<0.05).Conclusion The skin defect of skin flap transplantation for early intervention of hyperbaric oxygen therapy combined with intensive nursing intervention can improve patients with skin flap, skin traits significantly shorten the time for improvement of symptoms of patients, improve the success rate of transplantation and cosmetic satisfaction improved significantly.%目的:探讨在皮肤缺损皮瓣移植术后应用高压氧进行早期干预治疗,评估其临床治疗效果.方法:选择本院2 011年1月-2013年8月皮肤缺损皮瓣移植术后患者50例,早期均应用高压氧进行治疗,依据高压氧治疗时护理干预措施不同分为对照组和观察组,每组25例,对照组患者行常规护理,观察组在对照组基础上进行强化护理干预;10d后观察并进行临床治疗效果评估、皮肤症状消失及外观美容满意度评估.结果:观察组和对照组临床疗效和皮瓣坏死面积结果比较,差异具有统计学意义(P<0.05);术后l0d皮瓣生长状况评分和外观美容满意度均高于对照组,差异具有统计学意义(P<0.05);观察组患者皮肤红润改变时间、肿胀渗出消退时间结果均低于对照组,差异具有统计学意义(P<0.05).结论:皮肤缺损皮瓣移植术后高压氧早期干预治疗并配以强化护理干预,能够改善患者移植皮瓣的性状、明显缩短患者皮肤症状改善时间,提高移植成功率,外观美容满意度显著提高.
