唇侧固定正畸矫治后牙釉质脱矿相关因素Logistic分析及美容修复处理

         

摘要

Objective To investigate the related factors of enamel demineralization after orthodontic treatment of labial side fixation and the clinical efficacy of infiltration resin in the treatment of enamel demineralization. Methods The clinical data of 150 patients treated with orthodontic treatment of labial side fixation from January 2014 to May 2015 in our hospital were analyzed retrospectively. According to the occurrence of enamel demineralization after treatment, the patients were divided into demineralization group and non-demineralizing group, and there were 64 cases in the demineralization group, and there were 86 cases in the non-demineralizing group. The risk factors of enamel demineralizationwere analyzed by single factor descriptive analysis and multivariate Logistic linear regression. In addition, 64 patients with enamel demineralization were randomly divided into fluoride group (fluoride treatment) and infiltration resin group (infiltration resin therapy), and there were 32 cases in each group, and the clinical efficacy and cosmetic effect of the two groups were compared and analyzed. Results There was no significant difference of the rate of enamel demineralization between male and female patients after orthodontic treatment of labial side fixation (P >0.05), but the rates of enamel demineralization of the patients age ≤ 18 years, the times of treatment >16 months, brushing frequencies ≤3 times / d, without the use of fluoride toothpaste and with bad eating habitswere significantly higher than those of the patients age >18 years, the times of treatment≤16 months, brushing frequencies>3 times / d, the use of fluoride toothpaste and without bad eating habits (P <0.05),and the OR values of the age≤18years,the times of treatment>16 months, brushing frequencies ≤ 3times / d, without the use of fluoride toothpaste and with bad eating habits were respectively 2.37, 3.26, 4.35, 2.19 and 2.58, whichwerethe risk factors for enamel demineralization (P <0.05). The total effective rate of the infiltration resin group was 96.88%, which was significantly higher than 81.25% of the fluoride group (P<0.05). The excellent and good rate of cosmetic result of the infiltration resin group was was 96.88%, which was significantly higher than 81.25% of the fluoride group (P <0.05). Conclusion The occurrence of enamel demineralization after orthodontic treatment of labial side fixation is significantly related to the age of patients, the course of treatment, frequency of tooth brushing, the type of toothpaste and the bad eating habits, which are the risk factors of occurrence of enamel demineralization, and there are theobvious clinical effect and better cosmetic effect that the treatment of enamel demineralization by infiltration resin.%目的:探讨唇侧固定正畸矫治后牙釉质脱矿的相关影响因素及渗透树脂治疗牙釉质脱矿的临床疗效.方法:回顾性分析2014年1月-2015年5月在笔者科室进行唇侧固定正畸矫治的150例患者病例资料,根据治疗后牙釉质脱矿发生情况将其分为脱矿组和未脱矿组,其中脱矿组64例,未脱矿组86例,采用单因素描述性分析和多因素Logistic线性回归分析法分析相关危险因素.此外,将64例牙釉质脱矿患者随机分为氟化物组(氟化物治疗)与渗透树脂组(渗透树脂治疗),每组32例,对比分析两组患者的临床疗效与美容效果.结果:唇侧固定正畸矫治后男女患者的牙釉质脱矿发生率无明显差异(P>0.05),但年龄≤18岁、治疗时间>16个月、刷牙频率≤3次/d、未使用含氟牙膏、有不良饮食习惯患者的脱矿发生率均明显高于年龄>18岁、治疗时间≤16个月、刷牙频率>3次/d、使用含氟牙膏、无不良饮食习惯患者(P<0.05),且年龄≤18岁、治疗时间>16个月、刷牙频率≤3次/d、未使用含氟牙膏及有不良饮食习惯的OR值分别为2.37、3.26、4.35、2.19、2.58,均是牙釉质脱矿发生的危险因素(P<0.05);渗透树脂组总有效率为96.88%,显著高于氟化物组的81.25%(P<0.05);渗透树脂组美容效果优良率为96.88%,显著高于氟化物组的81.25%(P<0.05).结论:唇侧固定正畸矫治后牙釉质脱矿的发生与患者的年龄、治疗时间、刷牙频率、牙膏类型及不良饮食习惯有显著关系,均是牙釉质脱矿发生的危险因素,且牙釉质脱矿发生后给予渗透树脂治疗,临床效果显著,且有较好的美容效果.
