不同年龄段正常前列腺DTI研究

             

摘要

目的:利用DTI技术定量分析不同年龄段正常前列腺外周带(peripheral zone,PZ)和中央腺区(central zone,CZ)各向异性值(fractioanl anisotropy,FA)和ADC值,研究其随年龄的演变规律,利于前列腺疾病早期发现和诊断. 方法:50名志愿者根据年龄分为3组,A组14例15~30岁,B组24例31~50岁,C组12例≥51岁,分别测量和分析不同年龄段正常前列腺PZ和CZ的FA和ADC值的差异. 结果:各年龄段PZ左右叶FA值、ADC值差异均无统计学意义(均P>0.05). 各年龄段PZ的FA值均低于CZ,PZ的ADC值均高于CZ(均P<0.05).PZ的FA值及ADC值不同年龄段差异均有统计学意义(均P<0.05).CZ的FA值及ADC值不同年龄段的差异均无统计学意义(均P>0.05). Pearson相关分析结果显示PZ的FA值与年龄呈负相关(r=-0.498, P<0.05),ADC值与年龄呈正相关(r=0.682,P<0.05),CZ的FA值和ADC值变化与年龄均无明显相关性(均P>0.05). 结论:DTI定量分析能够反映前列腺随年龄变化的差异,为前列腺疾病的早期发现和诊断提供参考依据.%Objective:The focus of this study was to quantify age-related changes of fractional anisotropy (FA) and apparent diffusion coefficient ( ADC ) in the normal prostate peripheral zone ( PZ ) and central zone ( CZ ) using MR diffusion tensor imaging (DTI). Then,to establish the normal reference range,which would help early detection and diagnosis of prostate disease. Methods:50 healthy volunteers were divided into 3 groups according to different age:group A (15~30 years,n=14),group B (31~50 years,n=24),group C (>51 years,n=12). The FA and ADC values of PZ and CZ were measured. Difference between parameters of the PZ and CZ were assessed. Results:There was no significant difference of the FA and ADC values between the left and right of PZ (P>0.05). There were significant differences of the FA and ADC values on PZ and CZ in each group (P<0.05). The FA and ADC values on PZ in different age groups were (0.227 ±0.052), (0.202 ±0.055), (0.145 ±0.034);(1.439±0.160)×10-3mm2/s,(1.652±0.256)×10-3mm2/s,(2.001±0.266)×10-3mm2/s,respectively. There were significant differences of FA and ADC values on PZ between these groups (P<0.05). The FA and ADC values on CZ in different age groups were 0.291±0.08,0.287±0.045,0.257±0.059;(1.374±0.171)×10-3mm2/s,(1.382±0.178)×10-3mm2/s,(1.415±0.136)×10-3mm2/s,respec-tively. There was no significant difference of FA and ADC values on CZ among these groups (P>0.05). Pearson analysis sug-gested that the FA values on PZ were negative correlation with ages (r=-0.498,P<0.05),and the ADC values on PZ were pos-itive correlation with ages (r=0.682,P<0.05). There was no correlation between the changes of FA and ADC values and age on CZ. Conclusion:DTI quantitative measurement can reflect age-related changes of normal prostate. The normal reference range is established,which provide reference for early detection and diagnosis of prostate disease.
