3.0 T BOLD-fMRI评价灯盏花素对大鼠对比剂急性肾损害的抵制作用

         

摘要

Objective:To evaluate the protective effect of breviscapine for contrast agent induced acute kidney injury on rats using 3 . 0T blood oxygen level dependent ( BOLD ) magnetic resonance imaging technology . Methods:The 60 SD rats were randomly divided into control group and breviscapine group (each n=30) respectively to the injection of contrast agent before and after continuous injection of Breviscapine and normal saline 7 d (one time everyday),BOLD-fMRI was performed on rats of each before and 20 min,24 h,48 h,72 h and 7 d after injection of contrast agent,measuring rats renal outer medullary R2*values,and pathological examination in kidney of rats. After HE staining evaluation outer medullary tubular damage,analyzed and compared two groups of R2* value and pathological changes. Results:Control group rats renal outer medullary RBF value injection of contrast agent after 20 min R2* values increased,continued to 72 h,7 d after dropped to near the injection of contrast agents. Breviscapine group contrast injection 20 min R2* values continued to increase to 24,48 after down continued to 7 d. Injection of contrast agent after 48 h and 72 h breviscapine group and control group the difference was statistically significant (P<0.05),the other difference was no statistical significance (P>0.05). Pathological results showed that control group injection of contrast agent after 20 min renal outer medullary kidney tubules damaged,reduced 24 h to 48 h after injury,72 h increased,but less than 20 min,7 d after the injury was aggravated,similar to the group of 20 min;breviscapine group injection of contrast agent after 20 min of renal outer medullary kidney canaliculus serious injury,reduce 24 h to 48 h after injury and gradually lower than that of the control group,the degree of injury,72 h was aggravated,but lower than control group,7 d was alleviated. After contrast injection,there was no significant difference (P>0.05) between the two groups except the 24 h group,and the difference was statistically significant (P<0.05). Conclusion:3.0T BOLD technology can evaluate CIAKI induced renal blood perfusion level,reflect the treatment response of breviscapine injection on renal damage in CIAKI rats to a certain extent.%目的:应用3.0 T BOLD-fMRI评估灯盏花素对大鼠对比剂急性肾损害(CI-AKI)的抵制作用.方法:将60只SD大鼠随机分为灯盏花素组和对照组各30只,灯盏花素组于注射对比剂前后连续注射灯盏花素注射液7 d(1次/d),对照组于注射对比剂前后连续注射生理盐水7 d.2组于注射对比剂前及注射对比剂后20 min、24 h、48 h、72 h和7 d共6个时间点行3.0 T BOLD-fMRI扫描,测量大鼠肾脏外髓部R2*值,取大鼠肾脏病理检查,HE染色后评价外髓部肾小管损伤情况,分析比较2组R2*值及病理损伤变化.结果:灯盏花素组注射对比剂20 min后R2*值持续升高至24 h,48 h回落直至7 d时;对照组注射对比剂20 min后R2*值升高,持续至72 h,7 d后回落至注射对比剂前.注射对比剂后48、72 h灯盏花素组与对照组R2*值比较差异均有统计学意义(均P<0.05),余时间点2组R2*值比较差异均无统计学意义(均P>0.05).病理结果表明,对照组注射对比剂后20 min肾脏外髓部肾小管出现损伤,24~48 h损伤减轻,72 h损伤又加重,但低于20 min时,7 d后损伤加重,接近20 min组;灯盏花素组注射对比剂后20 min肾脏外髓部肾小管损伤严重,24~48 h损伤减轻,渐低于对照组损伤程度,72 h损伤又加重,但低于对照组,7 d后损伤减轻.注射对比剂后,24 h时2组损伤差异无统计学意义(P>0.05),余时间点2组差异均有统计学意义(均P<0.05).结论:3.0 T BOLD-fMRI可评估CI-AKI引起的肾髓质氧含量的变化,提供定量的R2*值,在一定程度上反映灯盏花素注射液对大鼠CI-AKI的早期保护作用.
