三维能量多普勒定量分析在卵巢肿瘤诊断中的应用价值

摘要

Objective To explore the values of quantitative three - dimensional power doppler sonography ( 3D -CPA ) in differential diagnosis of benign and malignant ovarian tumors. Methods The vessels index ( VI ), flow index ( FI ),vascularization - flow index ( VFI ) and the type of vessels were detected by 3D - CPA in 108 patients ( 123 tumors ) with ovarian tumors. The results were compared with postoperative pathological results. Results VI, FI, and VFI values in malignant tumors were significantly higher than in benign tumors [ ( 0.043 ±0.019 ) vs ( 0.011 ±0.006 ), ( 0.650 ±0.230 ) vs ( 0.180±0.086 ), ( 0.021±0.016 ) vs ( 0.008 ±0.004 ); P <0.05 ]. The sensitivity and specificity of diagnosing malignant ovarian tumor by criteria of VI≥0.04 , FI≥0.60, VFI≥0.02 were 90.3% and 82.2% , 89.1% and 80.5% , 86.3% and 82.9% respectively. Ⅲ ~ Ⅳ types of vessels accounted for 90.0% in malignant tumors , while Ⅰ - Ⅱ types of vessels accounted for 77.8%in benign tumors, with significant differences between the two groups ( P < 0.05 ). Conclusion Quantitative 3D - CPA can be effective in differentiating benign or malignant ovarian tumors.%目的 探讨三维能量多普勒超声(3D-CPA)定量分析对鉴别卵巢肿瘤良恶性的意义.方法 选取108例(123个肿瘤)卵巢实性或囊实性肿瘤患者,均进行3D-CPA检查,测定肿瘤的血管指数(VI)、血流指数(FI)、血管-血流指数(VFI),并进行血管分型,与术后病理结果对照.结果 卵巢恶性肿瘤的VI(0.043±0.019)、FI(0.650±0.230)、VFI(0.021±0.016)均显著高于良性肿瘤的VI(0.011±0.006)、FI(0.180±0.086)、VFI(0.008±0.004),卵巢良恶性肿瘤上述指标比较差异均有统计学意义(P<0.05).以VI≥0.04诊断卵巢恶性肿瘤的敏感度为90.3%,特异度为82.2%;以FI≥0.60诊断卵巢恶性肿瘤的敏感度为89.1%,特异度为80.5%;以VFI≥0.02诊断卵巢恶性肿瘤的敏感度为86.3%,特异度为82.9%.卵巢恶性肿瘤Ⅲ~Ⅳ型血管占90.0%,良性肿瘤Ⅰ~Ⅱ型血管占77.8%,卵巢良恶性肿瘤患者血管分型比较,差异有统计学意义(P<0.05).结论 3D-CPA定量分析是诊断卵巢肿瘤良恶性的有效方法.
