Objective To explore the model of home - based care for the elderly and its effectiveness. Methods A model of home - based care for the elderly was established and applied in Taoyuan Community in Shenzhen, Guangdong Provicne.The management and control of chronic non - communicable disease ( NCD ) were investigated using a questionnaire - based survey among 150 elderly people before and after the application of the model in June 2009. Results The satisfaction rate was increased hy 18. 7％ among elderly people after the implementation of the model. The control rate of hypertension and diabetes were significantly increased ( P ＜ 0. 05 ) . Conclusion Community - based care efficiently integrates the community resources and provide comprehensive and acceptable services for the elderly , and thus promotes the basic public health services in the community.%目的 探索桃源社区养老服务模式及其效果,为社区居家养老的发展提供科学依据.方法 建立社区动员体系,成立社区自我健康管理协会,利用社区资源为社区老年人提供居家养老服务.对比分析2009年6月开展居家养老服务前后社区老人慢性病的管理及控制情况,以及接受居家养老服务的社区老年人满意度变化情况.结果 桃源社区开展居家养老服务模式后,社区老年人高血压、糖尿病的管理率和控制率均得到了明显提高(P＜0.05),社区老年人满意度增加了18.7%(P＜0.01).结论 社区居家养老服务模式充分整合了社区可利用资源,为社区居民提供综合、全面的居家养老服务,社区居民乐于接受,进一步促进了社区基本公共卫生服务工作的提升.
