Objective To investigate the causes of two cases of primary hypoparathyroidism misdiagnosed as coronary artery heart disease and dilated cardiomyopathy. Methods The clinical manifestation and treatment of two patients with primary hypoparathyroidism in Peking University Third Hospital were analyzed retrospectively. Relevant literature was reviewed. Results Both two cases were mainly presented with dyspnea. One patient developed malignant arrhythmia, and the other suffered from convulsion. Both of them had atypical symptoms such as heart failure, unconsciousness, convulsion, and mini - myocardial infarction. They were finally confirmed as with primary hypoparathyroidism based on the clinical and laboratory findings. Conclusion Hypocalcium cardiomyopathy due to primary hypoparathyroidism should be considered for patients with heart failure and obvious hypocalcium, especially when the condition can not be explained by common heart diseases.%目的 分析2例原发性甲状旁腺功能减退症引起的低钙性心肌病分别误诊为冠状动脉性心脏病及扩张性心肌病的原因.方法 回顾性分析2例原发性甲状旁腺功能减退症引起的低钙性心肌病患者的临床表现和诊疗经过,并结合文献进行复习.结果 2例患者均以呼吸困难为主要表现,1例出现恶性心律失常,1例出现抽搐;2例症状均不典型,有心力衰竭、意识丧失、抽搐、微小心肌梗死等表现;均伴有明显的低钙血症.结合临床表现和实验室检查结果最终确诊为特发性甲状旁腺功能减退症.结论 临床医师对有心力衰竭合并低钙血症的患者,用常见的心脏疾病难以解释时,应想到原发性甲状旁腺功能减退症引起的低钙性心肌病的可能.
