危重患者院内转运流程再造的效果评价研究

摘要

Objective To explore reengineering the nosocomial transfer process for critically ill patients to improve the quality of medical service. Methods Panel points of transfer process were extracted by literature research methods and operational questionnaire designed. Field research method was used to find out existing problems in transfer process of critically ill patients , based on which a new transfer process was made and used clinically. Transit accidents, behavioral norm and satisfaction of medical staff were evaluated. Results After reengineering, transit accidents reduced, behaviors of medical staff became more prescriptive and all staff were satisfied with new process as compared with pre - reengineering. Conclusion Reengineered process, scientific and reasonable, can regulate effectively the medical staff's behaviors, thus to improve operational effects and reduce accident occurrence and enhance core competitiveness of hospitals.%目的 探讨危重患者院内转运流程再造的效果,提高医疗服务质量.方法 通过文献研究方法提取转运流程的节点并设计转运调查表,运用实地调研法找出我院现有危重患者转运流程中存在的问题,在此基础上设计新的转运流程,进行临床应用,并评价转运过程中意外发生情况、医务人员转运行为规范性及医务人员满意度.结果 流程再造前后输液管脱落或堵塞、供氧中断、呼吸困难、各种管道脱落发生率比较,差异均无统计学意义(P＞0.05).流程再造前后医务人员在没有通知目的的科室、分工不明确、站立位置有误和转运交接不清楚方面比较,差异均有统计学意义 (P＜0.05);两组医务人员在手术没有通知电梯、物品准备不完善、转运方式不合适、没有约束患者、转运人员不具资质和目的科室准备不充分方面比较,差异均无统计学意义(P＞0.05).结论 再造后的危重患者转运流程科学、合理,能有效规范医务人员行为,提高转运效率,减少转运意外事件发生,增强医院核心竞争力.
