无保护会阴分娩技术的临床应用效果探讨

摘要

Obj ective:To probe into clinical effect of unprotected perineum de-livery technology.Methods:Women receiving unprotected perineum delivery technology in our hospital from January to May in 2013 were taken as the ob-servation group,women receiving traditional delivery method to protect the perineum from January to May in 2012 were selected as the control group, midwives recorded and compared degree of episiotomy and laceration,post-partum hemorrhage,postpartum urinary retention,neonatal asphyxia be-tween both groups.Results:observation group episiotomy rate and perineum II degree laceration rate is lower than the control group,the perineum Com-plete rate and perineum I degree laceration is higher than the control group, two groups of women has not been a perineal laceration Ⅲ degrees,two groups of maternal perineum crack as a whole Inj ury degree to compare difference was statistically significant (χ2=294.042,P<0.01).Observation group of maternal postpartum urinary retention occurred obviously Lower than the control group,the difference was statistically significant (χ2=5.450,P<0.05 ).Two groups of maternal postpartum hemorrhage and neonatal as-phyxia By comparison,there was no statistically significant difference (χ2=0.396,P>0.396).Conclusion:unprotected perineum delivery technology can significantly reduce episiotomy rate in maternal,perineal laceration and oc-currence of postpartum urinary retention,effectively guarantee the maternal reproductive health and promote natural delivery.%[目的]探讨无保护会阴分娩技术在临床中的应用效果。[方法]对照组产妇采用常规保护会阴分娩法接生,观察组产妇采用无保护会阴分娩法接生,助产士分别记录并比较两组产妇会阴侧切及裂伤程度、产后出血、产后尿潴留、新生儿窒息情况等。[结果]观察组会阴侧切率及会阴Ⅱ度裂伤率低于对照组,会阴完整率及会阴Ⅰ度裂伤高于对照组,两组产妇均未出现会阴Ⅲ度裂伤,两组产妇总体会阴裂伤程度比较差异有统计学意义(χ2=294.042,P<0.01)。观察组产妇产后尿潴留发生明显低于对照组,差异有统计学意义(χ2=5.450, P<0.05)。两组产妇产后出血及新生儿窒息情况比较,差异无统计学意义(χ2=0.396,P>0.05)。[结论]无保护会阴分娩法能明显降低产妇会阴侧切率,减少会阴裂伤,减少产后尿潴留的发生,有效保障产妇生殖健康,促进自然分娩。
