社区护士在社区高血压管理中功能职责及实施路径

摘要

[目的]探讨社区护士在社区高血压管理中的功能职责及实施路径，以指导开展高血压社区控制。[方法]将150例社区高血压病人随机分为两组，对照组仅行常规社区干预，观察组制订护理路径管理表格，明确护士功能职责后行临床路径管理。对比两组干预前后健康知识得分、体质量指数、血压、空腹血糖水平、血压控制率及服药依从率。[结果]与对照组相比观察组干预后健康知识得分明显高于对照组、体质量指数明显更低、舒张压及收缩压明显更低、空腹血糖明显更低、血压控制率明显更高、服药依从率明显更高，差异均有统计学意义(P<0.05)。[结论]明确社区护士功能职责后，制订护理路径表格并严格执行，有助于提升社区高血压控制效果。%Objective:To probe into the functional responsibilities and implementation route of community nurses in community hypertension management,so as to guide the community hypertension control.Methods:A total of 150 cases of patients with community hypertension were randomly divided into two groups,75 cases in each. The patients in control group were given routine community intervention.The patients in observation group were given clinical pathway management by using nursing pathway management form and after identifying the functional responsibilities of nurses.The health knowledge score,body mass index,blood pressure,fasting blood glucose,blood pressure control rate and drug compliance were compared between both groups before and after intervention.Results:Health knowledge score of patients in observation group after intervention was signifi-cantly higher than that in control group.Body mass index was significantly lower,diastolic blood pressure and systolic blood pressure were significantly lower,fasting blood glucose was significantly lower,blood pressure control rate was significantly higher,compliance rate was significantly higher,and the differences were statisti-cally significant (P<0.05).Conclusion:After definiting the functional responsibilities of community nurses to make nursing pathway form and strictly carrying out can be helpful to improve the control effect of community hypertension.
