脑出血急性期血肿扩大影响因素研究

摘要

Objective:To explore the influence factors of enlargement of hematoma in the acute phase of intracerebral hemorrhage,and to provide evidendce for therapy.Methods:146 cases of cerebral hemorrhage which were divided into hematoma expansion group (41 cases) and control group (105 cases) were retrospective studied.The two groups of patients with the general situation,history of drinking,clinical features,radiographic examination,biochemical tests were statistically analyzed.Results:The difference of SBP in the next day after onset,the time from onset to first CT,the shape of hematoma,the blood HDL level between the two groups were statistically significant(P<0.05),but the difference of gender,age,history of drinking,the first SBP at admission,the first DBP at admission,etc,were not statistically significant(P>0.05).Conclusion:SBP in the next day after onset,the time from onset to first CT,the shape of hematoma, the blood HDL level are the influence factors of enlargement of hematoma in the acute phase of intracerebral hemorrhage.%　　目的：探讨血肿扩大的影响因素，为制定干预措施提供依据。方法：选择146例自发性脑出血(ICH)急性期患者，出现血肿扩大者41例为研究组，未出现血肿扩大者105例为对照组。对两组患者一般情况、饮酒史、临床特征、影像学检查、生化检查进行统计学分析。结果：两组发病次日收缩压、首次CT检查距发病时间、血肿形态、血高密度脂蛋白(HDL)水平比较差异有统计学意义(P<0.05)，而性别、年龄、饮酒史、入院首次收缩压、入院首次舒张压等比较差异无统计意义(P>0.05)。结论：发病次日收缩压、首次CT检查距发病时间、血肿形态、血高密度脂蛋白水平是自发性脑出血急性期血肿扩大的影响因素。
