不同剂量阿托伐他汀对家族性高胆固醇血症的降脂疗效评价

摘要

目的：探讨阿托伐他汀的不同剂量对杂合子家族性高胆固醇血症(familial hypercholesterolemia ，FH)的降脂效果。方法：选取家族性高胆固醇血症患者68例，随机分为观察组和对照组，每组34例。对照组每天给予阿托伐他汀片剂40 mg，观察组每天给予阿托伐他汀片剂60 mg，检测两组空腹外周血总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)的含量，并比较用药前后的疗效。结果：治疗一个疗程后，观察组TC、HLD-C、LDL-C分别为(5.12±0.64)mmol/L、(1.25±0.02)mmol/L、(3.13±0.48)mmol/L，与治疗前比较，差异有统计学意义(t=2.898，P<0.05；t=1.762，P<0.05；t=2.954，P<0.05)；对照组服药后TC、HLD-C、LDL-C分别为(7.34±0.32)mmol/L、(1.13±0.21)mmol/L、(4.06±0.21)mmol/L，与治疗前比较，差异有统计学意义(t=1.958，P<0.05；t=1.926，P<0.05；t=1.968，P<0.05)；治疗后，观察组TC、LDL-C、HDL-C下降水平明显优于对照组，差异有统计学意义(P<0.05)。观察组临床总有效率为91.18%(31/34)，对照组临床总有效率为82.35%(28/34)，两组比较差异有统计学意义(χ2=3.968，P<0.05)。结论：阿托伐他汀60 mg/d的剂量对杂合子FH的降脂效果理想，值得临床推广应用。%Objective:To investigate the lipid-lowering effect of Atorvastatin on heterozygous familial hypercholesterolemia(familial hypercholesterolemia,FH).Method:68 cases with heterozygous FH,were randomly divided into the observation group(n=34) and the control group(n=34).The observation group was given Atorvastatin tablets 60 mg,the control group was given Atorvastatin tablets 40 mg,detection of total cholesterol in each dosage group fasting peripheral blood(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C), efficacy was compared before and after treatment.Result:After one course of treatment,the observation group TC,HLD-C,LDL-C were (5.12±0.64) mmol/L,(1.25±0.02)mmol/L,(3.13 ±0.48)mmol/L,compared with before treatment,there were statistical significance(t=2.898,P<0.05;t=1.762,P<0.05;t=2.954,P<0.05).The control group after administration of TC,HLD-C,LDL-C were (7.34±0.32)mmol/L,(1.13±0.21)mmol/L, (4.06±0.21)mmol/L,compared with before treatment,there were statistical significance(t=1.958,P<0.05;t=1.926,P<0.05;t=1.968,P<0.05). The comparison between two groups,the observation group TC,LDL-C,HDL-C decreased significantly than that in the control group,with statistical significance(P<0.05).The total clinical effective rate of the observation group was 91.18%(31/34),the total clinical effective rate of the control group was 82.35%(28/34),there was statistically significant difference between two groups(χ2=3.968,P<0.05).Conclusion:The ideal lipid-lowering effect of Atorvastatin 60 mg/d dose of FH heterozygotes,is worthy of clinical application.
