我院门诊处方用药分析

摘要

Objective:Through analyzing outpatient prescription medication,statistics don't rational drug use,as well as the changing proportion of herbal medicine.Improving our rational drug use,more to speed up our national key specialty of Traditional Chinese Medicine at the grass-roots level of project construction.Method:From Jan 2012 to Dec 2012,choosing 121 510 zhang prescriptions from outpatient service center (Not including narcotic and psychotropic drugs prescription),according to《hospital prescription review management norms》,《prescription management method》instructions,to prescription for analysis and statistics.Result:The extraction in 2012,the year one over ten western medicine prescription 5853 zhang the unqualified prescription 614 zhang,accounting for 10.49%by pumping prescription mainly displays in the epilogue before prescribing remember don’t write complete,drip line,the body of the unqualified 352 zhang,of unqualified prescriptions for 57.33%(including compatibility taboo,improper combination,choose drug undeserved,repeated drug use,improper drug use frequency,no disease medication),14 370 cases,the number of the use of antimicrobial drugs account for 19.12%of the total number of prescription antibiotic prescription,using traditional Chinese medicine prescription number 72 980 cases.Traditional Chinese Medicine prescriptions accounted for 60.06%of the total number.Conclusion:Our hospital outpatient prescription is not rational drug use.Issued in the clinic doctors should strengthen the rational use of drugs prescription medicine to strengthen prescription audit,and ensure patient safety,economic and effective.The construction of national key specialty of Traditional Chinese Medicine project supporting the patients after the health care keeping in good health consciousness enhancement for Chinese medicine yinpian prescription dosage increasing.%目的：通过对门诊处方用药分析，统计不合理用药，以及中药用药比例变化，提高笔者所在医院合理用药，加快笔者所在医院基层中医民族重点专科项目建设。方法：抽取2012年1-12月笔者所在医院门诊中西药处方121510张(不包括麻醉和精神药品处方)，根据《医院处方点评管理规范》、《处方管理办法》、药品说明书，对处方进行分析和统计。结果：抽取2012年全年西药1/10处方5853张，其中不合格处方614张，占所抽处方的10.49%，主要表现在处方前记后记不写全、漏行、正文不合格352张，占不合格处方的57.33%(包括配伍禁忌、联用不当、选药不当、重复用药、用药次数不当、无指征用药)，使用抗菌药物人数14370例，抗菌药物处方占总处方的19.12%，使用中药处方人数72980例，中药处方占总处方的60.06%。结论：笔者所在医院门诊处方存在不合理用药情况。门诊医生应该加强合理用药处方开具，药师加强处方审核，保障患者用药的安全、经济、有效。基层中医民族重点专科项目建设扶持后患者保健养生意识增强，笔者所在医院中药饮片用量处方增加。
