后路切开复位内固定治疗胸腰椎爆裂型骨折的效果观察

摘要

Objective:To investigate the efficacy of posterior open reduction internal fixation treatment in thoracolumbar burst fracture.Method:Selected 50 thoracolumbar burst fractures patients from Feb 2013 to Feb 2014,all patients'spinal occupancy rate were more than 50% without obvious spinal cord injury symptoms.The clinical data and valued the treatment efficacy through imaging and social function were analyzed.Result:The average spinal occupancy rate was smaller than 10.9%,the average height of the vertebral body was near 90.5%.The Cobb's was averagely decreased 18.7°.Through follow-up form,patient's fracture healing was good,all resumed daily living skill and work ability.Conclusion:In treating thoracolumbar burst fracture,internal fixation treatment of posterior open reduction can effectively relieve symptoms and can restore vertebral height,canal volume and Cobb's without decompression,which can effectively avoid further damage in nerve or further deformity in spinal.So it has an important clinic value.%目的:分析和探讨后路切开复位内固定治疗胸腰椎爆裂型骨折的可行性与实效性.方法:选取2013年2月-2014年2月笔者所在医院收治的胸、腰椎爆裂型骨折患者共50例,全部病例术前经CT检查显示椎管占位率>50%,无明显脊髓损伤症状,回顾性分析采用后路切开复位内固定治疗的临床资料,并通过影像学和社会功能评价治疗效果.结果:术后密切观察患者相关临床指标,平均椎管占位率低于10.9%,平均椎体高度接近正常的90.5%,平均Cobb's角减小18.7°;采用随访形式,患者骨折愈合良好,均恢复日常生活能力,大多数患者恢复工作能力.结论:在治疗胸、腰椎爆裂型骨折中,采用后路切开复位内固定治疗手段,能有效地缓解临床症状,无需减压即可恢复椎体高度、椎管容积和Cobb's角,具有重要的临床价值.
