低体重婴幼儿先心病手术的体外循环经验

摘要

Objective:To summarize the management experience on cardiopulmonary bypass in infants below 10 kg with congenital heart disease operation in recent 5 years addamited in our hospital.Methods:123 cases of congenital heart disease were using low temperature shallow low temperature or extracorporeal circulation according to the degree of deformity complicated,and attach great importance to the protection of various organs.Results:The cardiopulmonary bypass time of the whole group was 25~188 min;aortic cross clamping time was 10~23 min;bypass Hct was 0.25~0.30;all of them were successfully offline.5 cases were dead after operation.Conclusion:The ECC management for infants with congenital heart disease should be as carefully as possible, in order to reduce the physiological disturbance of extracorporeal circulation on infants and promote postoperative recovery and reduce postoperative complications and mortality.%目的：总结本院5年来＜10 kg婴幼儿先天性心脏病的体外循环管理经验。方法：123例先天性心脏病，根据畸形复杂程度，采用浅低温或中低温体外循环，重视各种脏器的保护。结果：全组体外循环时间25～188 min，主动脉阻断时间10～123 min，转流中 Hct 0.25～0.30，均顺利脱机。术后死亡5例。结论：对于婴幼儿先天性心脏病的ECC管理力求做到精细，能够减轻体外循环对婴幼儿的生理干扰，促进术后恢复，减少术后并发症及死亡率。
