社区慢性病管理模式在老年高血压管理中的效果评价

摘要

Objective:To explore the clinical effect of elderly patients with hypertension after using community chronic disease management model.Methods:80 elderly patients with hypertension were selected.They were randomly divided into the two groups on average.The control group received routine basic intervention.The observation group used the community chronic disease management pattern.For blood pressure improvement,management satisfaction,the two groups of patients were analyzed.Results:After treatment,the blood pressure improvement of the observation group was significantly better than the control group(P<0.05). The satisfaction degree of the observation group(100% )was significantly higher than that of the control group(80% ),and the difference was statistically significant between groups(P<0.05).Conclusion:Elderly patients with hypertension using community-based management of chronic diseases achieved remarkable clinical results,not only improving patients’ satisfaction with management,while improving blood pressure levels significantly.%目的：探究老年高血压患者运用社区慢性病管理模式后所取得的临床效果。方法：收治老年高血压患者80例，随机平分两组。对照组采用常规基础干预，观察组应用社区慢性病管理模式。针对血压改善、管理满意度情况，对两组患者进行对比分析。结果：观察组治疗后血压改善情况明显优于对照组(P＜0.05)；观察组满意度(100%)显著高于对照组(80%)，两组差异存在统计学意义(P＜0.05)。结论：老年高血压患者运用社区慢性病管理模式后所取得的临床效果显著，在提高患者对管理工作满意度的同时，使患者血压水平显著改善。
