去眶隔脂肪组织的眼袋整形

摘要

Objective To explore a new blepharoplasty which is easier to operate and more effective than the old ones.Methods A total of 163 cases undergone blepharoplasty were taken as research subjects. The method of skin flap was adopted through incision in margin of palpebra inferior, then the orbital fat, weakened septum orbital, partial orbicularis oculi and loose skin were properly excised. Ocular orbicularis and orbital septum are jointed with the orbital periosteum by means of suture. The effect was observed after the surgery.Results All the 163 cases of blepharoplasty with intensifying orbital septum and removing partial adipose tissue, had received satisfying curative effects.Conclusion The balance between the adipose tissue and supporting tissue of sockets of eyes was reconstructed. It can prevent not only from the bad operative effect for not enough excised adipose tissue, but also from the sunken sockets of eyes for too much excised adipose tissue. The surgery is simple and easy to conduct, and it can provide stable curative effect.%目的：尝试为眼袋整形寻求一种新的手术方式，使手术变得简便而疗效稳定。方法眼袋整形手术患者163例为研究对象。下睑缘切口，皮下分离形成皮瓣，于眼轮匝肌眶部打开眶隔，适当切除眶隔内脂肪组织，梭形切除部分薄弱的眶隔筋膜及眶前眼轮匝肌，将其与下眶缘骨膜缝合以便加强眶前支持结构。观察手术效果。结果163例眼袋患者采取了部分去除眶隔内脂肪，加强眶前组织的手术，均获得了满意的疗效。结论眶内脂肪组织适当部分切除，同时将眶隔筋膜和眶前眼轮匝肌与眶缘骨膜固定，恢复了眶隔内组织压力与下睑眶隔支持结构之间的正常平衡状态。加强支持结构不但可以有效地防止因眶隔脂肪组织切除不够而造成的疗效不佳，而且也可预防由于眶内脂肪组织去除过多而出现的眶下凹陷，手术简单容易掌握而且疗效稳定。
