手术切除结合原位微波消融灭活术治疗骶骨脊索瘤的临床疗效

摘要

目的：探讨手术切除结合原位微波消融灭活术在骶骨脊索瘤治疗中的临床效果。方法6例骶骨脊索瘤患者,采用手术切除结合原位微波消融灭活术治疗,平均随访27.4个月,对手术入路、术中出血的控制、局部肿瘤病灶的处理、骶神经根的保护及术后并发症等进行回顾性分析。结果本组S3及以下肿瘤4例, S2肿瘤2例,6例患者均未出现肺转移及死亡病例。结论手术切除是骶骨脊索瘤最主要的治疗方法,术中出血的控制及骶神经根的保护是手术的关键,化疗及放疗效果不确切,局部结合微波消融灭活取得了满意的临床效果,值得临床借鉴。%Objective To investigate the clinical effect of resection combined with microwave ablation in situ inactivation method in the treatment of sacral vertebrae chordoma.Methods There were 6 patients with sacral vertebrae chordoma treated by resection combined with microwave ablation in situ inactivation method, with the average follow-up as 27.4 months. Their surgical approach, intraoperative bleeding control, local tumor lesion treatment, sacral nerve root protection, and postoperative complications were all retrospectively analyzed. Results There were 4 cases with S3 and lower level of tumor, and the other 2 cases with S2 level of tumor. No pulmonary metastasis and death cases occurred in the 6 cases.Conclusion Resection is the main treatment method for sacral vertebrae chordoma. Intraoperative bleeding control and sacral nerve root protection are the keys to operation. Although chemotherapy and radiotherapy provide imprecise effect, local combination of resection and microwave ablation inactivation can provide satisfactory clinical effect. This method is worthy of clinical reference.
