门诊处方评价与结果分析

摘要

目的 规范处方管理,提高处方质量,促进合理用药.方法 临床药师每月抽查门诊处方120张,按照<处方管理办法>、<抗菌药物临床应用指导原则>的有关规定,对所查处方进行点评,填写<处方评价表>,进行统计分析.结果 2009年1至12月共抽查门诊处方1440张,平均每张处方用药品种数为2.7种,抗菌药物使用率为54.5%,注射剂使用率为41.6%,基本药物占处方用药的72.7%,药品通用名占处方用药的85.7%,平均每张处方金额为71.27元.平均单处方药品种数超过5种者占当月处方的2.9%,未使用通用名者占当月处方的14.3%,缺临床诊断者占当月处方的13.1%.结论 我院处方书写还存在不少问题,应加大对<处方管理办法>的贯彻执行力度和门诊处方检查力度,使处方书写进一步规范化,不断提高临床用药水平.%Objective To improve the qualification of prescription and to promote the rational use of drugs.Methods One hundred and twenty prescriptions were analyzed by clinical pharmacists every month to accomplish routine evaluation and analysis which consist prescription indication and writing quality according to "Prescription Management Measure", "Guiding Principles for Clinical Application of Antibacterial Drugs" and "Henan Standard for Evaluation of Unqualified Prescription". Results On average, 2.7 drugs were used in each prescription. The percentage of antibiotics, injections, national essential drugs and drugs in chemical names were respectively 54.5%, 41.6%, 72.7% and 85.7%. The number of single prescription that containsmore than five kinds of drugs was 2.9%; prescriptions without using chemical names was 14.3% and the percentage of missing clinical diagnosis was 13.1%. Conclusion Pharmacists should strictly follow the Prescription Management Measure.
