急性冠状动脉综合征合并打鼾患者阻塞性睡眠呼吸暂停低通气综合征认知度改善的研究

摘要

目的 探讨不同宣教模式对急性冠状动脉综合征(ACS)合并打鼾患者对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)疾病认知度的改善效果.方法 选取首都医科大学附属北京安贞医院急诊危重症中心2016年1-8月收治的ACS患者,采用Berlin睡眠质量评估问卷筛选出864例合并打鼾患者作为研究对象,完全随机分为常规宣教组和强化宣教组,各432例,常规组患者给予常规护理宣教,强化宣教组在常规宣教的基础上给予多次多形式的宣教,即利用多媒体和OSAHS健康教育手册等方式实施宣教,比较不同宣教模式对2组患者OSAHS认知度的改善情况.结果 常规宣教组和强化宣教组患者性别、年龄和Berlin睡眠质量评估问卷高危比例差异无统计学意义(P>0.05);强化宣教组出院时及出入院OSAHS调查问卷评分差值大于常规宣教组[(5.7±2.4)分比(4.7±2.1)分、(2.7±1.9)分比(1.9±1.4)分],差异均有统计学意义(均P<0.01).结论 强化宣教对于改善患者对OSAHS的认知度明显优于常规宣教.加强对ACS合并打鼾患者的健康教育及宣传普及OSAHS知识,能够提高患者对打鼾的正确认识,减少疾病带来的严重危害.%Objective To explore effects of different educational models on improvement of obstmctive sleep apnea hypopnea syndrome (OSAHS) awareness in acute coronary syndrome (ACS) patients with snoring.Methods Patients with ACS were enrolled admitted from January 2016 to August 2016 in Emergency and Critical Care Center,Beijing Anzhen Hospital,Capital Medical University;864 snorers were selected by the Berlin sleep quality assessment questionnaire and randomly divided into routine education group (n =432) and intensive education group(n =432).Patients in routine education group received conventional nursing education;Patients in intensive education group received comprehensive,multi-forms and multi-media OSAHS health education.Awareness improvement of OSAHS in patients was analyzed.Results Gender,age and high risk ratio of Berlin questionnaire had no significant differences between groups(P > 0.05).The difference value of OSAHS questionnaire score at admission and discharge in intensive education group was significantly higher than that in routine education group[(5.7±2.4)points vs (4.7 ±2.1)points,(2.7 ± 1.9)points vs(1.9 ± 1.4)points] (P<0.01).Conclusion Strengthening health education can significantly improve awareness of OSAHS in ACS patients with snoring and reduce serious harm caused by the disease.
