目的：颈动脉内膜中层厚度（CIMT）是一种对亚临床动脉粥样硬化进行评估的无创操作。本研究目的是研究妊娠糖尿病妇女的CIMT。方法30例妊娠糖尿病妇女作为实验组，40例正常妇女作为对照。空腹8h后抽血测定空腹血糖、胰岛素、同型半胱氨酸、总胆固醇、高密度脂蛋白、甘油三酯、低密度脂蛋白和极低密度脂蛋白水平，同时测定CIMT。结果实验组甘油三酯水平较对照组升高（P=0.016），两组胆固醇、HDL、LDL和胰岛素水平无差别；实验组同型半胱氨酸、空腹血糖水平明显升高（P=0.027，P=0.019）；妊娠糖尿病患者CIMT高于正常对照[（0.582±0.066）mm vs（0.543±0.049）mm，P=0.006）]。CIMT与孕妇年龄（r=0.317，P=0.008)、BMI（r=0.437， P=0.007)、同型半胱氨酸（r=0.341，P=0.008)、空腹血糖（r=0.258，P=0.041)和OGTT试验时1h血糖水平（r=0.452， P=0.001)正相关。CIMT与妊娠糖尿病之间有明显正相关关系（r=0.368，P=0.001)。结论与正常妇女相比，妊娠糖尿病患者颈动脉内膜中层厚度增加。%Objective Common carotid artery intima-media thickness(CIMT) is an non-invasively assessed marker of subclinical atherosclerosis, Our aim in this study was to investigate CIMT in women with gestation diabetes mellitus. Methods Thirty women with GDM and forty unaffected women(as a control group) were included in the study. Blood samples were drawn from each woman in the morning after they had fasted for at least 8hour,and levels of fasting gluco se,insulin,homocysteine,total cholesterol, HDL cholesterol,triglycerides, LDL cholesterol were measured,along with the CIMT in the two groups. Results The mean triglycerides(P=0.016)levels in the GDM group was significantly higher than that in the control group.There were no significant differences between the groups with respect to plasma levels of total cholesterol, HDL, LDL and insulin. The mean homocysteine(P=0.027) and fasting glucose(P=0.019) levels in women with GDM were significantly higher than those in the unaffected women. Patients with GDM had significantly higher CIMT than did the unaffected women[(0.582±0.066mm vs 0.543±0.049mm,P=0.006)].CIMT correlated positively with maternal age(r=0.317,P=0.008)、BMI、(r=0.437,P=0.007)、homocysteine levels(r=0.341,P=0.008)、and fasting glucose(r=0.258,P=0.041)、and 1-h glucose value(r=0.452,P=0.001) at the time of the oral glucose tolerance test. There was a positive correlation between the presence of GDM and CIMT(r=0.368,P=0.001). Conclusion Women with GDM have increased CIMT compared with unaffected women.
