目的及时掌握布氏杆菌病的流行动态，为制定防制措施提供依据。方法对确诊187例布氏杆菌病患者进行流行病学调查，并对临床资料进行分析，采用布氏杆菌凝聚试管法进行抗体滴度检测。结果确山县布氏杆菌病呈现暴发流行态势，病例主要集中在该县竹沟镇（占51.4%），向周边乡镇辐射；患者以30～60岁青壮年居多，占81.3%，平均年龄（45.4±4.7）岁，男女比为3.16︰1。每年4~5月份为发病高峰期，职业分布以梳绒感染为主；23.2%有接触史的患者在接触时采取防护措施；临床特点以发热、乏力、多汗、肌肉酸痛、骨关节痛、食欲减退为主要表现，关节受累以腰、膝负重关节为主，7.5%的患者伴有肝功能异常。结论确山县及其周边地区布氏杆菌病疫情呈现局部暴发流行，应加大布氏杆菌病的防控力度。%Objective To grasp timely the epidemic dynamics of brucellosis in order to provide the basis for the establishment of prevention and control measures. Methods The epidemiological investigation and clinical data analysis of 187 cases with brucellosis were carried out. The antibody titer was tested by brucella condensation tube method. Results Brucellosis presented outbreak epidemic situation in Queshan county. The cases were mainly concentrated in Zhugou town of the county(51.4%),and radiated to the surrounding villages and towns. The patients were mostly 30 to 60 years young that accounted for 81.3%. The average age was(45.4±4.7) years old. The sex ratio of male and female was 3.16︰1. The onset of peak time were appeared April to May each year. Many of the infected persons mainly engaged in cashmere comb. Only 23.2%of patients with a history of exposure took protective measures. The main clinical manifestation were fever,fatigue,sweating,muscle aches,bone and joint pain and loss of appetite. The joint involvement were mainly in the waist,knee-based weight-bearing joints. 7.5%of the patients had abnormal liver function. Conclusion It shows that brucellosis appear localized outbreaks in Queshan county and its surrounding areas. It is necessary to strengthen the prevention and control of brucellosis.
