BNP及高敏CRP在COPD急性加重期合并肺动脉高压中的表达及临床意义

         

摘要

ObjectiveTo discuss expression and significance of BNP and hs-CRP in acute exacerbation of COPD with pulmonary hypertension. Methods Clinical data of 48 cases with acute exacerbation of COPD were respectively analyzed. All cases were divided into group of normal pulmonary artery pressure, group of mild pulmonary hypertension and group of moderate to severe pulmonary hypertension according to pulmonary artery pressure. BNP, hs-CRP and PaO2 of three groups were compared. Relationship between BNP, hs-CRP and PaO2 with pulmonary hypertension was analyzed.ResultsBNP, hs-CRP and PASP levels of group of normal pulmonary artery pressure were lower than group of mild pulmonary hypertension and group of moderate to severe pulmonary hypertension (P<0.01); BNP, hs-CRP and PASP levels of group of mild pulmonary hypertension were lower than group of moderate to severe pulmonary hypertension(P<0.01). BNP showed a significant positive correlation with hs-CRP and PASP, and a negative correlation with PaO2(P<0.01); hs-CRP showed a significant positive correlation with PASP, and a negative correlation with PaO2(P<0.01); PaO2 showed a significant negative correlation with PASP(P<0.01).ConclusionBNP and CRP levels in acute exacerbation of COPD with pulmonary hypertension rise with the increase of the pulmonary artery pressure.%目的:探讨BNP及高敏CRP在COPD急性加重期合并肺动脉高压中的表达及临床意义。方法回顾性分析COPD患者48例的临床资料,根据患者的肺动脉压分为肺动脉压正常组,轻度肺动脉高压组,中重度肺动脉高压组,比较三组患者血浆BNP、hs-CRP以及PaO2水平,并分析BNP、hs-CRP以及PaO2与肺动脉压的相关性。结果肺动脉压正常组患者BNP、hs-CRP、PASP水平显著低于轻度肺动脉高压组和中重度肺动脉高压组(P<0.01);轻度肺动脉高压组患者BNP、hs-CRP、PASP水平显著低于中重度肺动脉高压组(P<0.01)。BNP与hs-CRP、PASP呈显著正相关的关系,与PaO2呈显著负相关的关系(P<0.01);hs-CRP与PASP呈显著正相关的关系,与PaO2呈显著负相关的关系(P<0.01);PaO2与PASP呈显著负相关的关系(P<0.01)。结论 COPD急性加重期合并肺动脉高压患者BNP及CRP水平随着肺动脉压的升高而升高。
