老年冠心病患者氟伐他汀强化降脂的疗效及安全性观察

摘要

Objective: To discuss the efficacy and safety on Fluvastatin sodium (lescol) in the treatment of old patients with coronary heart disease (CHD) complicated by hyperlipidemia. Methods: All 60 elderly patients with CHD complicated by hyperlipidemia were treated by Fluvastatin sodium for 40 mg/d and got the desired value. They were randomly divided into treatment group and control group, with 30 cases in each group. The treatment group was given Fluvastatin sodium for 20 mg/d, while the control group stopped Fluvastatin sodium. At the same time, all patients were given the conventional treatment. The indexes such as blood-fat and C-reactive protein, the attack rates of both heart-failure and angina cordis, the adverse reactions on liver function and kidney function were all observed after treating for 3 and 6 months. Results: The indexes of TC, TG, LDL-C and hs-CRP all declined significantly, the HDL-C rised significantly (P<0.05), after treating for 3 and 6 months. While in the control group, the indexes of TC, TG, LDL-C and hs-CRP increased and the HDL-C declined obvi -ously (P<0.05). There were significant differences on the attack rates of heart-failure and angina cordis between the two groups (P<0.05). Conclusion: The intensive treatment of Fluvastatin sodium in treating elderly patients with CHD complicated by hyperlipidemia is effective, safe and is deserved to be widely used.%目的:探讨氟伐他汀钠(来适可)强化治疗冠心病(CHD)伴高脂血症老年患者的疗效及安全性.方法:所有60例冠心病伴高脂血症的老年患者,均经过氟伐他汀钠40 mg/d治疗后血脂已达标.随机分为两组,各30例.治疗组继续予以氟伐他汀钠20 mg/d;对照组停用氟伐他汀钠.所有患者治疗期间均按常规治疗.分别观察3、6个月后血脂、C反应蛋白水平变化;记录心力衰竭、心绞痛发作情况;观察肝、肾功能损害等不良反应发生情况.结果:治疗组治疗3个月和6个月后,总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)及超敏C反应蛋白(hs-CRP)均有明显下降,高密度脂蛋白(HDL-C)显著上升(P<0.05).对照组观察3个月和6个月后,TC、TG、LDL-C及hs-CRP均较前明显升高,HDL-C明显下降(P<0.05).治疗6个月后,对照组心衰加重、心绞痛发作例数与治疗组比较,差异均有统计学意义(均P<0.05).结论:应用氟伐他汀强化治疗老年冠心病伴高脂血症,疗效显著,安全性好,值得临床推广.
