无创机械通气治疗呼吸衰竭的临床应用进展

     

摘要

Objective: To investigate clinical effect of non-invasive mechanical ventilation in the treatment of chronic ob -structive pulmonary disease (COPD) combined with respiratory failure. Methods: 52 patients of COPD combined with respiratory failure were divided into treatment group (26 cases treated with non-invasive mechanical ventilation) and control group (26 cases treated with traditional treatment). PaO2, PaCO2, heart rate (HR), respiratory rate (R), complication, death cases and total length of hospital stay of two groups were analyzed and compared. Results: R、HR、PaO2、PaCO2、Ph value of the two groups at different times were compared, and there was no significant difference between them (P>0.05). The complication, death cases, total length of hospital stay and the time of improving blood and gas of treatment group were significantly better than those of control group (P<0.05). Conclusion: Noninvasive mechanical ventilation can support alveolar ventilation, improve and maintain oxygenation, relieve respiratory muscle fatigue, and it can significantly reduce the incidence of complications, so that remission rate and satisfaction rate of patients is very high.%目的:探讨无创机械通气脱机治疗慢性阻塞性肺疾病(COPD)并呼吸衰竭临床疗效.方法:选取采用无创机械通气治疗的26例COPD并呼吸衰竭患者作为治疗组,选择同期采用传统方式治疗26例COPD并呼吸衰竭患者作为对照组,分析并比较两组患者机械通气前后动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)、呼吸频率(R)、心率(HR)、并发症、死亡例数及住院时间.结果:两组不同时间段治疗前后R、HR、PaO2、PaCO2、pH值比较,差异均无统计学意义(P>0.05).治疗组并发症、死亡例数、住院时间及血气改善时间均小于对照组,差异有统计学意义(P<0.05).结论:无创机械通气能够达到支持肺泡通气、改善并维持氧合、缓解呼吸肌疲劳的目的,可显著降低并发症的发生,患者缓解率及满意度高.
