2型糖尿病合并颈动脉斑块的相关危险因素分析

摘要

目的 探讨2型糖尿病(T2DM)合并颈动脉粥样斑块形成的危险因素.方法 将2010年1月~2012年1月北京军区总医院收治的258例2型糖尿病患者根据超声诊断结果分为斑块组及非斑块组,对两组患者的年龄、性别、腰围、体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FBG)、餐后2 h血糖(2 h BG)、糖化血红蛋白(HbA1c)、胰岛素敏感指数(ISI)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)进行测定及分析.结果 258例T2DM患者中共检出合并颈动脉粥样斑块86例,颈动脉斑块的检出率达33.3%.单因素分析表明年龄、SBP、2 h BG、HbA1c、ISI、TG、LDL-C等因素均具有相关性;多元回归分析表明年龄、SBP、2 h BG、ISI、TG是T2DM患者发生颈动脉粥样斑块的独立危险因素.结论 年龄、SBP、2 h BG、ISI、TG是引起2型糖尿病患者合并颈动脉斑块的独立危险因素,在临床治疗过程中应注意对具有上述危险因素的患者进行控制及预防,防止脑血管事件的发生.%Objective To investigate the risk factors for type 2 diabetic patients with carotid artery plaque. Methods 258 patients with type 2 diabetes treated in Beijing Command General Hopital from January 2010 to January 2012 were divided into plaque group and the none-plaque group based on ultrasound diagnosis. age, sex, waist circumference, body mass index (BMI), systolic blood pressure (SBP), diastolic pressure (DBP), fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h BG), glycated hemoglobin (HbA1c), insulin sensitivity index (TST), and triglycerides (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) of patients were measured and analyzed. Results 86 cases of 258 patients were diagnosed with combination of carotid atherosclerotic plaques, and the carotid artery plaque detection rate was 33.3%. Univariate analysis showed that age, SBP, 2 h BG, HbAlc, ISI, TG, LDL-C and other factors were relevant to carotid artery plaque; multiple regression analysis showed that age, SBP, 2 h BG, ISI, TG were independent risk factors leading to carotid artery plaque for T2DM patients. Conclusion Age, SBP, 2 h BG, ISI, TG are independent risk factors leading to carotid artery plaque for T2DM patients, and patients with the such risk factors need control and prevention in the clinical treatment, in order to prevent cerebrovascular events.
