系统护理干预在髓内钉治疗胫骨骨不连中的临床应用

         

摘要

目的 探究系统护理干预在髓内钉治疗胫骨骨不连中的临床应用.方法 选取哈尔滨医科大学附属第一医院2014年7月~2016年7月收治的90例接受髓内钉治疗的胫骨骨不连患者,按照就诊时间顺序分为对照组(2014年7月~2015年7月)与观察组(2014年8月~2015年7月),每组各45例,对照组仅给予常规护理,观察组给予系统护理干预,均护理2个月,对比两组患者Barthel指数、视觉模拟评分(VAS)、负性情绪及护理满意度.结果 两组护理后Barthel指数较护理前升高,VAS较护理前降低,观察组改善较对照组更加显著,差异有统计学意义(P<0.05).两组护理后汉密顿焦虑量表及汉密顿抑郁量表评分均较护理前降低,观察组改善较对照组更加显著,差异有统计学意义(P<0.05).对照组护理满意度为75.56%,低于观察组的93.33%,差异有统计学意义(P<0.05).结论 系统护理干预相比于常规护理可促进改善髓内钉治疗胫骨骨不连患者的临床效果,改善日常生活能力,降低疼痛感及负性情绪,提高护理满意度.%Objective To explore clinical application of system nursing intervention in intramedullary nailing in the treatment of tibial bone nonunion.Methods 90 patients with tibial bonenonunion treated with intramedullary nailing in the First Affiliated Hospital of Harbin Medical University from July 2014 to July 2016 were selected.All patients were divided into control group (from July 2014 to July 2015) and observation group (from August 2014 to July 2015) according to registration order,45 cases in each group.Routine nursing was used in control group,system nursing intervention was applied in observation group.After nursing for 2 months,Barthel index,VAS,negative emotion and nursing satisfaction between two groups were compared.Results After nursing,Barthel indexes of two groups were higher than before nursing,VAS of two groups were lower than before nursing,observation group was improved significantly than control group,with statistical difference (P < 0.05).After nursing,HAMA and HAMD scores of two groups were lower than before nursing,observation group was improved significantly than control group,with statistical difference (P < 0.05).Nursing satisfaction rate of control group was 75.56%,lower than that of observation group (93.33%),with statistical difference (P < 0.05).Conclusion The system nursing intervention can improve clinical effect of intramedullary nailing in the treatment of tibial bone nonunion,improve daily living ability,reduce the pain and the negative emotion,improve nursing satisfaction.
