Objective To observe the effect of Amiodarone combined with Yangxin Dingji Capsules on cardiac function and vascular endothelial function in patients with chronic heart failure and ventricular premature beat.Methods One hundred and ten cases with chronic heart failure and ventricular premature beat admitted to the Second Hospital of Hebei Medical University from March 2016 to March 2017 were selected and divided into control group and research group by random number table method,with 55 cases in each group.Control group was given conventional Amiodarone treatment.Research group was given Yangxin Dingji Capsules on the basis of control group,the course was eight weeks.The clinical effects,heart function before and after treatment,changes of endothelial function and the conditions of adverse reactions in the two groups were observed.Results The total effective rate of research group was 89.1％,which was significantly higher than that of control group (72.7％) (P ＜ 0.05).After treatment,the levels of left ventricular ejection fraction in the two groups were all significantly increased compared with those before treatment,while the levels of left ventricular end-diastolic diameter,left ventricular end systolic diameter,QT dispersion degree,serum NT-proBNP were significantly lower than those before treatment,and the improvement of cardiac function indices in the research group was obviously better than those in the control group (P ＜ 0.05).After treatment,the levels of NO in the two groups were significantly increased compared with those before treatment,while the levels of ET-1 were significantly lower than those before treatment,and the improvement of vascular endothelial function indices in the research group was significantly better than those in the control group (P ＜ 0.05).Conclusion Amiodarone combined with Yangxin Dingji Capsules in the treatment of patients with chronic heart failure and ventricular premature beat has better clinical effect,which can effectively improve the number of premature beat,ventricular function and vascular endothelial function,and it is worthy of clinical promotion and application.%目的 观察胺碘酮联合养心定悸胶囊对慢性心力衰竭合并室性早搏患者心功能及血管内皮功能的影响.方法 选取2016年3月～2017年3月河北医科大学第二医院收治的慢性心衰合并室性早搏的患者110例,按照随机数字表法分为对照组和研究组,每组各55例.对照组给予常规胺碘酮治疗,研究组在对照组的基础上给予养心定悸胶囊,疗程为8周.观察两组的临床疗效、治疗前后心功能、血管内皮功能变化及不良反应发生情况.结果 治疗后,研究组总有效率(89.1％)显著高于对照组(72.7％)(P＜0.05).治疗后,两组患者左室射血分数水平均较治疗前显著升高,而左室舒张末期直径、左室收缩末期直径、QT离散度、血清N-末端脑钠肽前体水平均较治疗前显著降低,且研究组的心功能指标改善情况优于对照组,差异有统计学意义(P＜ 0.05).治疗后,两组患者一氧化氮水平均较治疗前显著升高,而内皮素-1水平均较治疗前显著降低,且研究组血管内皮功能指标改善情况优于对照组,差异有统计学意义(P＜ 0.05).结论 胺碘酮联合养心定悸胶囊治疗慢性心力衰竭合并室性早搏的临床效果优于单独使用胺碘酮,其能有效改善患者早搏次数、心室功能及血管内皮功能,值得进行推广使用.
