目的：了解河北省围场县居民缺血性脑血管病病因构成，为有针对采取干预措施提供依据。方法选择近3年在该院住院的资料完整脑梗塞患者459例作为研究对象。由两名有经验的神经内科医师依据临床资料和血管评估结果进行 CISS 分型。结果459例缺血性卒中患者中，大动脉粥样硬化性占63.4%，穿支动脉病变占18.1%，心源性卒中占11.32%。其他原因占2.6%。在其他原因的12例中，布氏杆菌神经性占5例。大动脉粥样硬化组与其他组比较，合并高血压、高血脂、吸烟、糖尿病等危险因素明显偏高。高血压在大动脉粥样硬化组发生率是78.45%，在穿支动脉病变组发生率为95.2%，但二者无显著性差异。糖尿病在二者之间也无显著性差异。结论大动脉粥样硬化性卒中也是该县缺血性脑血管病最常见原因，针对三高危险因素，低盐饮食、平衡膳食、适当运动，可减少动脉粥样硬化的发生。布氏杆菌病引起的神经系统损害也是该县缺血性脑血管病的一个原因。%Objective To understanding the pathogeny composition of ischemic cerebrovascular disease in residents in We-ichang country in Hebei province. To provide the basis for targeted intervention measures. Methods 459 cases of cerebral infarction inpatients with complete data in our hospital in recent 3 years were selected as the research objects, and were given CISS typing according to the clinical data and vessel assessment results by two experienced neurologic doctors. Re-sults In the 459 cases of patients with ischemic stroke, large-artery atherosclerosis accounted for 63.4%, penetrating artery disease accounted for 18.1%, cardioembolic stroke accounted for 11.32% and other reasons accounted for 2.6%, in the 12 cases due to other reasons, brucella nerve occurred to 5 cases, the risk factors of hypertension, hyperlipoidemia, smoking and diabetes in the large-artery atherosclerosis group were obviously high compared with those in the other group with sta-tistical significance, the incidence rate of hypertension was 78.45% in the large-artery atherosclerosis group and 95.2% in the penetrating artery disease group, but there was no obvious difference, and there was no obvious difference in diabetes between the two. Conclusion The large artery atherosclerosis stroke is the most common cause of ischemic cerebrovascular disease in our country, for three high risk factors, low-salt diet, balanced diet and appropriate exercise can reduce the oc-currence of atherosclerosis, and the nervous system damage caused by brucellosis is also one of the causes of ischemic cerebrovascular disease in our country.
