预见性护理在老年卧床压疮患者护理中的应用探析

摘要

目的：探讨预见性护理在老年卧床压疮患者护理中的作用。方法随机选取2012年8月—2014年8月在该院接受治疗的110例老年卧床患者为研究对象，随机数表法分为对照组与观察组，对照组50例行基础护理，观察组60例在随机护理基础上行预见性护理，比较两组患者的Braden量表评分值与护理中压疮发生情况及首发时间。结果两组患者在Braden量表评分中，对照组VS观察组：感觉[(2.2±0.3)分VS(3.5±1.8)分]、活动[(2.1±0.8)分VS(3.1±1.6)分]、移动[(2.4±0.5)分VS(3.6±1.1)分]、潮湿[(2.0±0.4)分VS(3.2±1.4)分]、营养[(2.5±0.6)分VS(3.3±1.2)分]、摩擦力与剪切力[(1.8±0.3)分VS(2.7±0.9)分]，观察组均显著高于对照组，对比差异具有统计学意义（P＜0.05）。观察组压疮患者总发生率5.0%显著低于对照组20.0%，组间差异显著；两组首次发生时间比较[(15.4±4.3)dVS(5.9±1.6)d]观察组显著高于对照组，差异有统计学意义（P＜0.05）。结论预见性护理对老年卧床压疮患者预防与恢复有重要作用。%Objective To investigate the effect of predictive nursing on elderly bedridden patients with pressure ulcer. Methods 110 elderly bedridden patients treated in our hospital from August 2012 to August 2014 were selected as the subjects and divided into the control group and the observation group in accordance with the random number table method. 50 cases in the control group were given basic nursing, and 60 cases in the observation group were given predictive nursing based on random nursing. Braden Scale score, incidence of pressure ulcer during nursing and the first onset time of pressure ulcer were compared between the two groups. Results Compared with the control group, in terms of Braden Scale score, the observation group had much higher sensory perception score[(3.5±1.8) vs (2.2±0.3)], obviously higher activity score[(3.1±1.6) vs (2.1±0.8)], significantly higher mobility score[(3.6±1.1) vs (2.4±0.5)], substantially higher moisture score[(3.2±1.4) vs (2.0±0.4)], much higher nutrition score[(3.3±1.2) vs (2.5±0.6)], obviously higher friction & shear score[(2.7±0.9) vs (1.8±0.3)] with statistically significant difference(P<0.05). The over-all incidence of pressure ulcer was much lower in the observation group than in the control group (5.0% vs 20.0%). The observa-tion group had much later first onset time of pressure ulcer than the control group[(15.4±4.3)d vs (5.9±1.6)d] with statistically sig-nificant difference (P<0.05). Conclusion Predictive nursing has an important role in the prevention and recovery of elderly bedrid-den patients with pressure ulcer.
