It studies the technology path and application research for chronic disease management of regional health information platform based on health case record, offers a mode of regional integrated collaborative service for chronic patients by making use of the platform of regional chronic disease business, realizes linkage of regional health management business, solves the problems such as information isolation and service isolation, and offers a beneficial research for the mode of chronic disease management based on regional health information platform.%研究了基于健康档案区域卫生信息平台的慢病管理技术路径,以及利用区域慢病业务平台对慢病患者提供区域一体化协同服务模式,实现区域健康管理业务联动,解决信息隔离、服务孤立等问题,为基于区域卫生信息平台的慢病管理模式提供了有益的探索.
