一种半自动的肝脏分段方法及三维可视化实现

     

摘要

肝脏分段是精准肝切除手术的解剖学基础,肝脏切除手术可以大幅度降低手术的发病率和减小术后的死亡率,被广泛采用。但是,由于肝内血管复杂多变,医生根据二维CT影像估计肝切除的位置仍然存在一定的风险。肝脏分段的三维可视化有助于医生术前全面了解肿瘤的位置、大小与周围血管的空间毗邻关系。根据Couinaud肝分段理论,并结合外科医生的实践经验,提供了一套肝分段的三维可视化系统。系统有效避免了由于肝内血管结构的多样性所导致的部分分段结果与解剖结构不一致的现象,为外科医生提供了一种快速、直观和有效的肝脏分段方法。%Liver segment is the theoretical basis of the precise hepatectomy. Liver resection is widely used, which can greatly reduce the incidence of postoperative surgery and the mortality rate. However, due to the complex intrahepatic vessels, surgeons still have some risk to image stereotaxic liver tumors and carry out the surgery according two dimensional CT images. The 3D visualization of the liver segments can help surgeons understanding the tumor's location, size, the spatial relationship with peripheral vascular structures. We propose a method to carry out liver classification and three dimension visualization, which is based on the theory of Couinaud's, combined with the experience of surgeons. The system can avoid inconsistencies between the segmentation results and the actual diversity of the liver due to the diversity of intrahepatic vascular anatomy and provide a more accurate preoperative assessment tools for clinical liver surgery.
