原发性高血压患者血压变异性与脑梗死的相关性研究

             

摘要

目的探讨原发性高血压患者血压变异性与脑梗死的关系,为改善原发性高血压患者预后提供参考。方法收集原发性高血压患者200例。根据是否存在脑梗死将患者分为A组(单纯高血压)72例, B组(高血压合并症状脑梗死)78例,C组(高血压合并有症状脑梗死)50例。比较三组患者的一般资料及血脂指标及24 h平均收缩压、舒张压(24 hSBP、24 hDBP);日间平均收缩压、舒张压(dSBP、dDBP);夜间平均收缩压、舒张压(nSBP、nDBP);血压变异性。结果各组TG、TC、LDL-C、HDL-C、ApoA、ApoB水平比较差异无统计学意义(P>0.05)。B组、C组24 hSBP、24 hDBP、dSBP、dDBP、nSBP和nDBP均高于A组(P<0.05),B组、C组的24 hSBPV、dSBPV均高于A组(P<0.05)。结论对原发性高血压患者,年龄较大者的24 h收缩压、日间收缩压变异性与脑梗死的发生密切相关,因此对于原发性高血压的治疗应重视患者的血压变异性,以降低脑梗死的发生风险,改善其预后。%Objective To investigate the relationship between blood pressure variability and cerebral infarction in patients with essential hypertension, and to provide reference for improving the prognosis of patients with essential hypertension. Methods 200 cases of essential hypertension patients were collected. According to the existence of cerebral infarction patients were divided into A group (simple hypertension) 72 cases, B group (hypertension complicated with cerebral infarction) in 78 cases, C group (hypertension with symptomatic cerebral infarction) in 50 cases. Compared three groups of patients with general information and biochemical indicators and 24 h average systolic blood pressure, diastolic blood pressure (24 hSBP, 24 hDBP), daytime average systolic blood pressure, diastolic blood pressure (dSBP, dDBP), nocturnal mean systolic blood pressure, diastolic blood pressure (nSBP, nDBP), blood pressure variation of. Results There were no significant differences in TG, TC, LDL-C, HDL-C, ApoA and ApoB levels between the groups (P>0.05). B group, C group 24 hSBP, 24 hDBP, dSBP, dDBP, Nsbp and nDBP were signiifcantly higher than the A group (P<0.05), B group, C group of 24 hSBPV, dSBPV were signiifcantly higher than the A group (P<0.05). Conclusion On primary hypertension patients, older age of 24 h blood pressure, daytime systolic blood pressure variability and cerebral infarction is closely related to the occurrence, therefore for primary hypertension treatment should pay attention to the patient's blood pressure variability and to decrease the cerebral infarction risk, improve the prognosis.
