螺旋CT双期增强扫描对胰腺癌的诊断价值

             

摘要

目的 评价螺旋CT双期增强扫描在胰腺癌诊断及评价中的价值.方法 分析22例术后病理证实胰腺癌的双期螺旋CT扫描图像,扫描包括平扫,动脉期(注射后20~25 s)和实质期(注射后50~60 s)扫描,测量各扫描时期肿瘤和正常胰腺组织的CT值.观察肿瘤与周围血管及脏器之间的关系.结果 肿瘤-胰腺正常组织之间的最大密度差为动脉期(70±15)HU,与平扫及实质期期密度差有显著差别(P<0.05).病灶与周围动脉之间的关系于动脉期显示清楚,与周围静脉及脏器之间的关系于实质期观察清楚.结论 动脉期时可获得肿瘤-胰腺密度最大差值,螺旋CT双期增强扫描可以满足临床对胰腺癌的诊断和评价.%Objective To assess the value of dual -phase imaging at spiral computed tomography (CT) for pancreatic carcinoma. Methods Twenty-two patients with pancreatic carcinoma underwent dual-phase spiral CT. Images were obtained at no-enhancement , arterial phase (after injection 20-50 seconds) , pancreatic phase (after injection 50 ~60 seconds) , which evaluated attenuation of tumor and normal pancreas, vascular and organs invasion. Results Mean tumor-to-gland attenuation difference was the greatest on images obtained in the arterial phase (70 ± 15) HU,significantly different from that on those obtained in the pancreatic phase and in the no-enhancement (P 〈 0. 05 ). The sensitivity of images obtained in the arterial phase for prediction of arterial invasion was significantly better than that of others phase, however pancreatic phase was significantly better for prediction of venous and organs invasion. Conclusion The maximum attenuation difference of tumor-to-gland can be obtained in arterial phase CT scan. Images of the pancreas obtained in the dual- phase with spiral CT has advantages in diagnosing and evaluating pancreatic carcinoma.
