进展期胃癌联合脏器切除术临床分析

摘要

Objecu've:To assess the effect and safety of combined evisceration for the treatment of advanced gastric carcinoma. Methods .The clinical data were reviewed in 58 advanced gastric carcinoma patients under gone treatment with combined evisceration between Jan. 2006 and Jun. 2010. Results:No perioperative death occurred. Yet,left subphrenic infection occurred in 2, postoperative infection in 4, incision disruption in 3, gastromotility disorder in 1, anastomosis leakage in 1, pancreatic fistula in 1 and incomplete ileus in another 1, who were cured. The follow-up suggested that one-year survival rate was 81.03% ,51.72% survived 2 years and 44.82% ,3 years. Conclusion: If the surgical indications are strictly controlled, treatment of advanced gastric carcinoma with combined evisceration could lead to longer survival and sound quality of life for those patients.%目的:探讨进展期胃癌的扩大根治手术中施行联合脏器切除的根治性与安全性.方法:对我院2006年1月～2010年6月期间施行联合脏器切除的58例进展期胃癌的临床资料进行回顾性分析.结果:无手术死亡病例.术后左膈下积液2例,切口感染4例,切口裂开3例,胃动力障碍1例,吻合口瘘1例,胰瘘1例,不全性肠梗阻1例,均治愈出院.随访1年生存率为81.03％；2年生存率为51.72％；3年生存率为44.82％.结论:严格选择手术适应证,联合切除受累脏器可提高胃癌的根治性、延长生存期并获得较好的生活质量.
