Objective: To determine the concentration ofcarbamazepine in epileptic patients plasma and saliva by TDX in fluorescence polarization immuno assay (FPIA), study their relativity ,inspect the possibility of using saliva instead of plasma in clinical monitoring of carmabazepine .Methods: The total number of patients was 31 cases ,who had orally taken of CBZ 200mg-1300mg/day. Serum and saliva concentrations at 0 hours and 24 hours were measured by TDX in fluorescence polarization immuno assay (FPIA), and their relativity was studied by statistic methods. RESULTS: The concentration of CBZ in saliva at 0hours and 24hours had a mean of 1.01±0.79 μg·ml-1 and 0.83±0.79 μg·ml-1respectively, the above values did not differ significantly from each other. The concentration of CBZ in plasma at 0 hours and 24hours had a mean of 2.39±1.37 μg·ml-1和 2.16±1.25 μg·ml-1respectively. The above values did not differ significantly from each other. The ratio of saliva concentration to plasma was (0.335±0.061).The relativity of the concentration of carmazepine in patients' plasma and saliva was quite good (the mean correlation coefficient was 0.659,P<0.0001 ).Conclusions: High relativity may exist between the concentrations of carbamazepine in saliva and serum which is dynamic and parallel. So it is feasible to use saliva instead of plasma in clinical monitoring of carmazepine .
