A systematic review and meta-analysis on Adjunctive effects of Nd:YAG laser in chronic periodontitis:Evaluation of clinical periodontal parameters
BACKGROUND Various beneficial characteristics of laser such as ablation or vaporization, bactericidal, sterilization, bio stimulation and hemostatic effects have led to the potential application in periodontal therapy. OBJECTIVE The aim of present study is to broaden the present concept on Nd:YAG laser application adjunct to scaling and root planning in treatment of chronic periodontitis patients by systematically evaluating the available evidence. MATERIAL AND METHODS Extensive electronic and manual search on the relevant topic was done till end of August 2016. All potential articles were reviewed by two individual reviewers and selected through consensus. A meta-analysis was conducted according to the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-analysis statement and the Cochrane Collaboration. Bias risk was evaluated according to Cochrane tool for risk of bias evaluation. Meta-analyses of the clinical attachment level gain (CAL), probing depth reduction (PD), gingival index (GI), Plaque index (PI), bleeding on probing (BOP) and gingival crevicular fluid (GCF) value were performed using weighted mean differences for continuous data with 95% confidence intervals in a random effect model in Revman 5.3. RESULTS Six potential publications were eligible for the systematic review out of which only fivewere included in meta-analysis. Low to unclear risk of bias was detected, but heterogeneity was significant among studies. Significant statistical changes were detected in terms of PD (p=0.003), BOP (p=0.03) and GCF (p<0.0001). CONCLUSION: The present literature demonstrated some positive findings for the adjunctive use of Nd:YAG laser in treatment of chronic periodontitis. However, the available evidence is insufficient to implement adjunctive Nd:YAG laser as a definitive treatment. So, further long term, well.
